明代新安医家孙一奎从天人四气论治眩晕病刍议

摘　　要：对《孙文垣医案》《赤水玄珠》《医旨绪余》中眩晕病的31则验案与论述进行分析,归纳出孙一奎从天人四气论治眩晕病的特色。对病因病机的阐述主要有脾土不燥,湿邪壅遏清窍;肾命虚衰,清气出入失常;金受火侮,肺气降令难行经隧不利;营气难养脑窍四个方面。治法方药包括三部兼顾,散邪通窍;佐金平木,肃肺降气;温补肾命,纳清降逆;补营荣脑,通调经隧。眩晕病用药禁忌包括忌用丹石镇坠;禁用燥热劫阴;少用味厚滋腻。孙氏从天人四气论治眩晕病丰富了中医杂病辨治理论,有助于发挥传统中医药治疗的优势。This paper analyzes the 31 cases of vertigo in SUN Wen Yuan Yi An,Chi Shui Xuan Zhu and Yi Zhi Xu Yu,and sums up the characteristics of SUN Yikui s treatment of vertigo from the four Qi of heaven and man.The etiology and pathogenesis of spleen soil is not dry,dampness obstructing Qingqi;Kidney deficiency and failure,Qing Qi access disorder;Gold fire insult,lung Qi down to make difficult to travel through the tunnel unfavorable;Ying Qi is difficult to maintain four aspects of brain orifice.Treatment methods and prescriptions include three parts,dispelling evil and clearing orificum;Warm tonifying kidney,nourrishing Ying and brain,through the channel tunnel.Vertigo drug contraindications include contraindications of Danshi;Prohibit dry heat to rob Yin;Use less thick and greasy.SUN s treatment of vertigo from the four Qi of heaven and man enriched the theory of differentiation and treatment of various diseases in traditional Chinese medicine,and contributed to the advantages of traditional Chinese medicine treatment.
