温针灸防烫伤烧损护理用具临床应用防护现状及影响因素分析

摘　　要：目的调查温针灸防烫伤烧损护理用具临床应用防护现状,分析影响安全防护因素,为用具改进提供依据。方法于2021年6月—2022年2月,采用方便抽样法将广州中医药大学深圳医院(福田)门诊灸疗室81例温针灸治疗患者作为调查研究对象,采用自制调查表对温针灸防烫伤烧损护理用具的临床应用防护状况现场观察记录,统计分析影响因素。结果温针灸防烫损现有护理用具为纸片、纸板和一次性针灸针外壳,皆为用废弃物自行剪制应用无相关规范做法;单个灸疗穴位用具使用平均数1.35,用具烧损发生率24.3%;方差分析显示用具材质不良、患者局部烫感呼叫、用具阻燃性能不良、插口不畅、插口狭窄摩擦针体、燃艾柱掉落是用具性能不良影响安全防护的相关因素(P<0.05);多因素Logistic回归分析显示,阻燃隔热性能不良、插口摩擦针体、燃艾柱掉落是用具安全防护风险因素(P<0.05)。结论温针灸防烫伤烧损护理用具是临床应用安全防护必备用品,材料结构性能不良会发生烫伤烧损引发不良事件,须规范制作、有效防护、保障安全。Objective To investigate the current status of clinical application protection of warm acupuncture anti-scald nursing appliances,analyze the influencing factors of safety protection,and provide a basis for the improvement of appliances.Methods From June 2021 to February 2022,81 patients treated with warm acupuncture in the outpatient moxibustion treatment room of our hospital were investigated by convenient sampling method,and the on-site observation records of the clinical application of warm acupuncture anti-scald and burn nursing tools were recorded by self-made questionnaire,and the influencing factors were statistically analyzed.Results The existing nursing tools for warm acupuncture to prevent scalding were paper pieces,cardboard and disposable acupuncture needle shells,all of which were self-cut from waste and applied without relevant normative practices.The average number of individual moxibustion acupuncture utensils was 1.35,and the incidence of utensil burn loss was 24.3%.ANOVA showed that poor material of utensils,local scalding calls of patients,poor flame retardant performance of utensils,poor sockets,narrow sockets and friction needles,and falling of moxa columns were the relevant factors affecting the safety protection of utensils with poor performance(P<0.05).Multivariate logistic regression analysis showed that poor flame retardant thermal insulation performance,socket friction needle body,and falling of moxa column were the risk factors of appliance safety protection(P<0.05).Conclusion Warm acupuncture anti-scald burn nursing utensils are essential supplies for safety protection in clinical applications,and adverse events caused by scald and burn damage will occur due to poor material structure and performance,and it is necessary to standardize production,effective protection and ensure safety.
