摘 要:绝经后骨质疏松症是一种由雌激素缺乏引发的全身性代谢性骨病。雌激素除了直接影响骨代谢外,还会通过铁过载、免疫失调、氧化应激及炎症反应等多种途径间接诱发骨质疏松。目前,临床上对于绝经后骨质疏松症的治疗没有特效药,一直使用钙剂、双膦酸盐类、降钙素、性激素补充剂及雌激素受体调节剂等维持治疗。外泌体是一种细胞释放的特殊囊泡,具有稳定性高、靶向性强、免疫原性低等特点,在细胞通讯过程中发挥重要作用,此种特性可用于多种疾病的治疗。为了解决绝经后骨质疏松症治疗的瓶颈问题,本文主要归纳总结了不同来源的外泌体对绝经后骨质疏松症的治疗作用,并探讨当前存在的问题和后续的发展方向,以期为绝经后骨质疏松症的基础研究和临床治疗提供参考。Postmenopausal osteoporosis is a metabolic bone disease caused by estrogen deficiency.Estrogen not only directly affects bone metabolism,but also indirectly induces osteoporosis through various pathways such as iron overload,immune dysfunction,oxidative stress and inflammatory response.At present,there is no specific drug for the treatment of postmenopausal osteoporosis in clinical.The conventional drugs have been used:calcium,bisphosphonates,calcitonin,sex hormone supplements,estrogen receptor modulators and other maintenance treatment.Exosomes are special vesicles released by cells,which have the characteristics of high stability,strong target and low immunogenicity.They play an important role in the process of cell communication,and these characteristics can be used in the treatment of a variety of diseases.In order to solve the bottleneck problem in the treatment of postmenopausal osteoporosis,this article mainly summarizes the therapeutic effects of exosomes from different sources on postmenopausal osteoporosis and discusses the current problems and future development directions,for providing reference for the basic research and clinical treatment of postmenopausal osteoporosis.
