摘 要:目的研究海马硬化的磁共振成像(MRI)的特点及病理表现。方法本研究挑选了60例被诊断为海马硬化的患者,同时做磁共振成像液体衰减反转恢复序列(FLAIR)、T_(2)WI、T_(1)WI及横断面的磁共振扫描等检查。结果60例患者中,6例为双侧海马硬化,54例为单侧海马硬化(左侧23例,右侧31例),均经手术病理结果证实。其中48个患侧磁共振成像表现为患侧海马萎缩变小;T_(2)WI(32侧)和FLAIR成像(35侧)呈高信号;8个患侧颞叶萎缩;17个患侧颞角扩大;12个患侧白质萎缩;29个患侧海马内部结构消失;20个患侧海马头部浅沟消失。结论磁共振成像对于海马硬化诊断具有重要价值,海马硬化常见的磁共振表现为海马萎缩,同时信号增高,T_(2)WI与FLAIR联合检查可提高海马硬化诊断的准确度和敏感度。Objective To study the magnetic resonance imaging(MRI)features and pathological findings of hippocampal sclerosis.Methods A total of 60 patients with hippocampal sclerosis were diagnosed as fluidattenuated inversion recovery(FLAIR),T_(1)WI,T_(2)WI and cross-sectional scan perpendicular to the long axis of the hippocampus.Results Results of the 60 patients,6 cases were bilateral hippocampal sclerosis and 54 cases were unilateral hippocampal sclerosis(23 cases on the left side,31 cases on the right),with all cases underwent surgical pathology.Sixty cases with 48 affected sides MRI showed atrophy of the affected hippocampus.T_(2)WI(32 sides)and FLAIR imaging(35 sides)showed high signal,8 sides temporal lobe atrophy,17 sides temporal horn enlargement,12 ipsilateral white matter atrophy,the inner structure of the 29 affected hippocampal disappears,and the 20 superficial hippocampus of the affected hippocampus disappears.Conclusion MRI has a high diagnostic value for hippocampal sclerosis.The common MRI manifestations of hippocampal sclerosis are hippocampus atrophy and hippocampal increased signal,and the combination of FLAIR and T_(2)WI can improve the accuracy and sensitivity of the diagnosis of hippocampal sclerosis.
