微粒皮移植术与邮票植皮技术治疗大面积烧伤的临床效果

摘　　要：目的 探究微粒皮移植术与邮票植皮技术治疗大面积烧伤的效果及对植皮成活率、创面愈合情况的影响。方法 回顾性选取2022年1月至2023年12月河南电力医院医治的83例大面积烧伤患者作为研究对象,依据手术方法分为邮票组(44例)、微粒组(39例),前者接受邮票植皮技术,后者接受微粒皮移植术。比较两组手术指标、植皮成活率、手术前后氧化应激[丙二醛(MDA)、白介素-1β(IL-1β)、超氧化物歧化酶(SOD)]、生活质量、并发症。结果 与微粒组相比,邮票组治疗费用较少,创面愈合时间、住院时间和手术时间较短(P<0.05);术后1周,两组植皮成活率(97.73%和82.05%)相比,差异有统计学意义(P<0.05);术后1周,与微粒组相比,邮票组血清MDA、IL-1β、SOD水平较低(P<0.05);术后2周,与微粒组相比,邮票组生活质量较好(P<0.05);两组并发症发生率相比(2.27%和7.69%),差异无统计学意义(P>0.05)。结论 采用邮票植皮技术能提高大面积烧伤患者植皮成活率,缩短创面愈合时间,减轻机体氧化应激反应,安全性好。Objective To explore the curative effect of micro skin grafting and stamp skin grafting in the treatment of large area burn and its influence on the survival rate of skin grafting and wound healing.Methods A total of 83 cases of patients with large area burn treated in Henan Electric Power Hospital from January 2022 to December 2023 were selected as the research object,and they were divided into stamp group(44 cases)and microparticle group(39 cases)according to different surgical methods.The former underwent stamp skin grafting technology,and the latter underwent microparticle skin grafting.The operation indexes and survival rate of skin graft were compared between the two groups,as well as oxidative stress[malondialdehyde(MDA),interleukin-1β(IL-1β),super oxide dismutase(SOD)],quality of life and complications before and after operation.Results Compared with the microparticle group,the stamp group had less treatment cost,shorter wound healing time,hospital stay and operation time(P<0.05).One week after operation,the survival rate of skin grafting in the two groups(97.73%vs 82.05%)was significantly different(P<0.05).One week after operation,compared with the microparticle group,the levels of serum MDA,IL-1βand SOD in the stamp group were lower(P<0.05).Two weeks after operation,the quality of life in the stamp group was better than that in the microparticle group(P<0.05).There was no significant difference in the incidence of complications between the two groups(2.27%vs 7.69%)(P>0.05).Conclusion Stamp skin grafting technique can improve the survival rate of skin grafting,shorten the wound healing time,reduce the body’s oxidative stress reaction and has good safety in patients with large area burn.
