改良Henry入路内固定术治疗桡骨远端骨折的效果

摘　　要：目的探讨改良Henry入路内固定术治疗桡骨远端骨折的效果及安全性。方法采用前瞻性研究,依据随机数字表法将濮阳市人民医院2021年12月至2022年12月收治的80例桡骨远端骨折患者分为两组,各40例。对照组接受传统Henry入路内固定术,观察组接受改良Henry入路内固定术,术后均随访6个月。比较两组围手术期指标、腕关节功能、患侧腕关节活动度及术后并发症情况。结果观察组手术时间、骨折愈合时间短于对照组,术中出血量少于对照组,术后6个月并发症发生率低于对照组(P<0.05)。术后6个月,两组腕关节功能优良率、患侧腕关节活动度比较,差异无统计学意义(P>0.05),但两组患侧腕关节活动度均高于术后6周(P<0.05)。结论改良Henry入路和传统Henry入路内固定术均可改善桡骨远端骨折患者腕关节功能及腕关节活动度,临床治疗效果相当,但改良Henry入路方式术中出血量更少,手术时间及骨折愈合时间较短,安全性更高。Objective To explore the effectiveness and safety of improved Henry approach internal fixation for the treatment of distal radius fractures.Methods A prospective cohort study was conducted,and 80 patients with distal radius fractures admitted to Puyang People’s Hospital from December 2021 to December 2022 were divided into two groups according to the random number table,40 patients in each group.The control group received traditional Henry approach internal fixation,while the observation group received improved Henry approach internal fixation.All patients were followed up for 6 months after surgery.The perioperative indicators,wrist joint function,affected wrist joint range of motion,and postoperative complications were compared between the two groups.Results The observation group had shorter surgical time and fracture healing time than the control group,and the intraoperative blood loss and incidence of postoperative complications at 6 months was lower than those in the control group(P<0.05).Six months after surgery,there was no statistical significant difference in the excellent rate of wrist joint function and the range of motion of the affected wrist joint between the two groups(P>0.05),but the range of motion of the affected wrist joint in both groups was higher than that at six weeks after surgery(P<0.05).Conclusion Both the improved Henry approach and traditional Henry approach internal fixation can improve wrist joint function and range of motion in patients with distal radius fractures,with comparable clinical treatment effects.However,the improved Henry approach results in less intraoperative bleeding,shorter surgical and fracture healing times,and higher safety.
