摘　　要：目的 观察他克莫司软膏联合308 nm准分子激光治疗结节性痒疹的效果。方法 选取驻马店市中心医院2020年8月至2022年7月就诊的结节性痒疹患者86例,以随机数字表法将其分为研究组(43例,采用他克莫司软膏联合308 nm准分子激光治疗)与对照组(43例,采用308 nm准分子激光治疗)。两组患者均治疗3个月。对比两组临床疗效、瘙痒持续时间评分、皮损大小评分、瘙痒程度评分、皮损数量评分、安全性及炎症因子水平。结果 研究组的总有效率高于对照组(P<0.05)。治疗后,两组瘙痒持续时间评分与皮损大小评分均降低(P<0.05),且研究组更低(P<0.05)。治疗后,两组瘙痒程度评分与皮损数量评分均均低于治疗前(P<0.05),研究组低于对照组(P<0.05)。治疗后,两组血清γ-干扰素(IFN-γ)、肿瘤坏死因子α(TNF-α)、白细胞介素-17(IL-17)水平均降低(P<0.05),研究组低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 他克莫司软膏联合308 nm准分子激光可改善结节性痒疹患者临床症状,疗效显著,可减轻机体炎症反应,且安全可靠。Objective To observe the effect of tacrolimus ointment combined with 308 nm excimer laser in the treatment of nodular pruritic rash.Methods A total of 86 patients with prurigoma nodosum treated in Zhumadian Central Hospital from August 2020 to July 2022 were selected and divided into study group(43 cases,treated with tacrolimus ointment combined with 308 nm excimer laser)and control group(43 cases,treated with 308 nm excimer laser)by random number table method.All patients in two groups were treated for 3 months.Clinical efficacy,itch duration score,lesion size score,itch degree score,number of lesions,inflammatory factor level and safety of the two groups were compared.Results The study group of total effective rate was higher than control group(P<0.05).After treatment,itch duration score and lesion size score were decreased in the two groups(P<0.05),and were lower in the study group(P<0.05).After treatment,the scores of itch degree and number of lesions in two groups were lower than before treatment(P<0.05),and those in the study group were lower than control group(P<0.05).After treatment,serum levels of interferon-γ(IFN-γ),tumor necrosis factorα(TNF-α)and interleukin-17(IL-17)in both groups were decreased(P<0.05),and those in the study group were lower than control group(P<0.05).There was no difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Tacrolimus ointment combined with 308 nm excimer laser can improve the clinical symptoms of patients with nodular pruritus,and has significant efficacy,can reduce the inflammatory response,and is safe and reliable.
