Value of High-Frequency Ultrasound Combined with Firefly and SMI in the Differential Diagnosis of Benign and Malignant Thyroid Nodules

摘  要:目的 探讨高频超声联合萤火虫+超微血管成像(SMI)技术对甲状腺结节(TN)良恶性的鉴别诊断价值。方法 回顾性选取2022年3月至2023年3月开封市人民医院收治的62例(74个结节)TN患者的临床资料,以病理结果为金标准,将结节分为恶性组(28个)与良性组(46个)。分析高频超声诊断TN的图像特征以及SMI诊断TN的血供,统计高频超声、萤火虫成像与SMI单独及联合诊断TN恶性的结果,并分析4种诊断方式的诊断效能。结果 两组在边界、回声、纵横比,钙化灶比较,差异有统计学意义(P<0.05)。两组在穿支血管、内部血管、周边血管、混合型血管比较,差异有统计学意义(P<0.05)。高频超声诊断TN恶性35个,良性39个;萤火虫成像诊断TN恶性40个,良性34个;SMI诊断TN恶性44个,良性30个;三者联合诊断TN恶性21个,良性53个。以病理结果为金标准,高频超声诊断TN恶性的灵敏度、特异度和准确度分别为71.43%、67.39%、68.92%,萤火虫成像诊断TN恶性的灵敏度、特异度和准确度分别为78.57%、60.87%、67.57%,SMI诊断TN恶性的灵敏度、特异度和准确度分别为89.29%、58.70%、70.27%,三者联合诊断TN恶性的灵敏度、特异度和准确度分别为67.86%、95.65%、85.14%,其特异度和准确度均高于单项诊断方式。结论 高频超声、萤火虫成像与SMI对TN良恶性均有一定的鉴别诊断价值,且三者联合鉴别诊断价值更高。Objective To explore the value of high-frequency ultrasound combined with firefly and superb microvascular imaging(SMI)in the differential diagnosis of benign and malignant thyroid nodules(TN).Methods A retrospective collection was performed on the clinical data of 62 patients with TN(74 nodules)admitted to Kaifeng People’s Hospital from March 2022 to March 2023.Taking pathological results as the golden standard,nodules were divided into malignant group(28)and benign group(46).The image characteristics of high-frequency ultrasound in diagnosis of TN,and the blood supply of TN by SMI were analyzed.The diagnostic results of TN malignancy by high-frequency ultrasound,firefly imaging,SMI and combined detection were statistically analyzed,and the diagnostic efficiency of four diagnostic methods was analyzed.Results There were significant differences in boundary,echo,aspect ratio and calcification foci between the two groups(P<0.05),and there were also significant differences in perforating vessels,internal vessels,peripheral vessels and mixed vessels between the two groups(P<0.05).High-frequency ultrasound showed that there were 35 malignant TN and 39 benign TN.Firefly imaging showed that there were 40 malignant TN and 34 benign TN.SMI showed that there were 44 malignant TN and 30 benign TN.The combined detection showed that there were 21 malignant TN and 53 benign TN.Taking pathological results as the golden standard,the sensitivity,specificity and accuracy of high-frequency ultrasound in the diagnosis of TN malignancy were 71.43%,67.39%,68.92%,firefly imaging were 78.57%,60.87%,67.57%,SMI were 89.29%,58.70%,70.27%and combined detection were 67.86%,95.65%,85.14%,respectively.The specificity and accuracy of combined detection were higher than those of single method.Conclusion High-frequency ultrasound,firefly imaging and SMI all have certain value in the differential diagnosis of benign and malignant TN,and the value of combined detection is higher.
