基于思维导图构建通气治疗重症肺炎新生儿的协作式“零缺陷”护理模式

摘  要:目的 探究基于思维导图构建通气治疗新生儿重症肺炎协作式“零缺陷”护理模式,并观察其临床应用效果。方法 本研究为前瞻性随机对照试验,纳入2021年3月至2023年3月平顶山市第一人民医院收治的102例通气治疗新生儿重症肺炎患儿,根据随机数表法分为对照组(51例)和研究组(51例)。对照组接受协作式“零缺陷”护理,研究组接受基于思维导图构建的协作式“零缺陷”护理模式。统计患儿(咳嗽、发热、肺啰音)消退时间,采用小儿危重病例评分(PCIS)评估患儿康复进程,统计患儿住院期间误吸、漏气、喉梗阻、皮肤损伤等风险事件发生情况。结果 研究组患儿咳嗽、发热、肺啰音消退时间均短于对照组(P<0.05)。干预7 d后,两组患儿PCIS评分均较干预前显著提高,且研究组高于对照组(P<0.05)。研究组患儿风险事件总发生率低于对照组,但组间差异无统计学意义(P>0.05)。结论 基于思维导图构建的协作式“零缺陷”护理模式可缩短通气治疗新生儿重症肺炎患儿症状消退时间,加快患儿康复进程,减少风险事件发生。Objective To explore the construction of a collaborative“zero defect”nursing model for ventilation treatment of severe neonatal pneumonia based on mind mapping,and to observe its clinical application effect.Methods This study was a prospective randomized controlled trial that included non-invasive positive pressure ventilation treatment for severe neonatal pneumonia in 102 children admitted to the First People’s Hospital of Pingdingshan from March 2021 to March 2023.They were divided into a control group(51 cases)and a study group(51 cases)according to the random number table method.The control group implemented collaborative“zero defect”nursing,while the study group implemented a collaborative“zero defect”nursing model based on mind mapping.The regression time(cough,fever,pulmonary rales)of the children was counted.The pediatric critical illness score(PCIS)was used to evaluate the rehabilitation process of children before and 7 days after intervention.The incidence of risk events such as aspiration,air leakage,laryngeal obstruction and skin injury during hospitalization were counted.Results The disappearance time of cough,fever,and lung rales in the study group was shorter than that in the control group(P<0.05).After 7 days of intervention,the PCIS scores of the two groups were significantly higher than those before intervention,and the study group was higher than the control group(P<0.05).The total incidence of risk events in the study group was lower than that in the control group,but there was no statistical significant difference between the groups(P>0.05).Conclusion The collaborative“zero defect”nursing model constructed based on mind mapping can shorten the time for symptom resolution in newborns with severe pneumonia treated with ventilation,accelerate the recovery process of the children,and reduce the occurrence of risk events.
