摘　　要：溃疡性结肠炎(Ulcerative Colitis,UC)是以结直肠慢性复发性炎症为病理基础,致使结直肠黏膜屏障持续损伤的一类自身免疫性疾病,以持续反复发作的腹泻、黏液脓血便及腹痛为主要临床表现,免疫功能失调是UC发病的核心环节。脾为后天之本、气血生化之源,亦是重要的免疫器官,脾功能的正常与否影响着机体免疫力。研究认为UC发病是以脾虚为本,湿邪、瘀毒等为标,病机虚实夹杂以致UC缠绵难愈。在免疫调节过程中,树突状细胞(dendritic cells,DC)是唯一可激活初始T细胞且体内功能最强的抗原提呈细胞,是免疫发生发展的关键,在免疫环节中的炎症反应中扮演主体角色。文章基于免疫视角,探讨从脾对DC进行调控干预治疗UC。Ulcerative colitis(UC)is an autoimmune disease characterized by chronic recurrent inflammation of the colon and rectum,resulting in persistent damage to the mucosal barrier.The main clinical manifestations include recurrent diarrhea,mucus and pus in the stool,and abdominal pain.Immune dysfunction is a core mechanism underlying the development of UC.The spleen is the foundation of nurture,the source of Qi and blood,and an important immune organ.The normal function of the spleen affects the body's immunity.It is believed that the onset of ulcerative colitis is based on spleen deficiency,dampness,stasis,etc.The combination of pathogenesis and reality leads to ulcerative colitis that is persistent and difficult to heal.In the process of immune regulation,dendritic cells(DC)are the only antigen-presenting cells that can activate primary T cells and have the strongest function in the body.They are the key to the development of immunity and play a major role in the inflammatory response in the immune process.This article explores the intervention of regulating DC from the spleen in the treatment of UC from an immunological perspective.
