健脾活血渗湿方联合恩替卡韦治疗早期乙肝肝硬化患者疗效及对免疫功能影响的相关研究

摘　　要：目的观察健脾活血渗湿方联合恩替卡韦对早期乙肝肝硬化患者的效果以及对免疫功能的影响。方法将杭州市西溪医院肝病科2023年1月—2024年6月我院住院诊断为代偿期乙肝肝硬化患者40例,随机软件分为治疗组和对照组,各20例。对照组给与恩替卡韦片,治疗组加用健脾活血渗湿方,比较两组治疗前后肝功能、肝纤维化指标、肝脏硬度、细胞免疫功能指标及中医证候积分变化情况,同时比较两组治疗期间的不良反应发生情况。结果治疗3个月后,治疗组ALT、AST、TBIL、HA、LN、Ⅳ-C、PⅢP,以及LSW均显著低于对照组(P<0.05);治疗组患者的A/G,CD4^(+)、CD4^(+)/CD8^(+)、CD16^(+)/CD56^(+)NK值明显高于对照组;治疗组中医证候积分更显著下降(P<0.05),两组入组后均无出现明显不良反应。结论健脾活血渗湿方联合恩替卡韦能恢复早期乙肝肝硬化患者的肝功能,降低肝纤维化指标和肝脏硬度,改善细胞免疫功能及中医临床证候,抑制肝纤维化的进程。Objective Observe the effects of Jianpi Huoxue Shenshi recipe combined with entecavir on patients with early hepatitis B sclerosis and the impact on immune function.Methods From January 2023 to June 2024,Hangzhou Xixi Hospital was diagnosed as 40 patients with hepatitis B sclerosis during compensation.The computer random machine was divided into the treatment group and control group,20 cases.The control group was treated with entecavir,and the treatment group was used to Jianpi Huoxue Shenshi recipe.Compare the two groups to treat the changes in liver function,liver fibrosis indicators,liver hardness,cellular immune function indicators,and Chinese medicine symbolism.The adverse reactions during treatment occurred.Results After 3 months of treatment,the treatment group Alt,AST,TBIL,HA,LN,IV-C,PIINP,and LSW were significantly lower than the control group(P<0.05);CD4^(+)/CD8^(+),CDL6^(+)/CD56^(+)NK value is significantly higher than that of the control group;the treatment group of traditional Chinese medicine syndrome equal points ratio decreased significantly before treatment(P<0.05),and there was no obvious adverse reaction during the treatment period.Conclusion The Jianpi Huoxue Shenshi recipe combined with entecavir can restore the liver function of patients with hepatitis B sclerosis,reduce liver fibrosis indicators and liver hardness,improve cellular immune function and clinical syndrome of Chinese medicine,and inhibit the process of liver fibrosis.
