肛瘘切挂术治疗高位肛瘘临床研究

摘　　要：目的观察中医新术式肛瘘切挂术治疗高位肛瘘的临床疗效。方法60例高位肛瘘患者,随机分为治疗组与对照组,各30例。治疗组采用肛瘘切挂术治疗,对照组采用低位切开高位挂线术治疗。对比两组术后疼痛、切口完全愈合时间、肛门功能及复发情况。结果治疗组术后疼痛视觉模拟评分法(VAS)评分(术后第3 d、第7 d、第14 d)均明显低于对照组,差异有统计学意义(P<0.01);治疗组切口完全愈合时间(28.77±1.43)d显著低于对照组的(32.03±2.86)d,差异有统计学意义(P<0.01)。治疗组术后7 d、术后6个月Wexner评分均低于对照组,差异有统计学意义(P<0.01)。两组术后复发率比较差异无统计学意义(P>0.05)。结论应用肛瘘切挂术治疗高位肛瘘可明显减轻患者术后疼痛,缩短切口愈合时间,保护患者肛门功能。Objective To analyze the clinical effect of new traditional Chinese surgery anal fistula resection and hanging in the treatment of high anal fistula.Methods A total of 60 patients with high anal fistula are randomly divided into a research group and a control group,with 30 cases in each group.The research group is treated with new fistula resection and hanging operation,and the control group with traditional low-incision and high-hanging operation.Postoperative pain,incision healing time,anal function and recurrence are compared between the two groups.Results The VAS score of the research group is significantly lower than that of the control group(on the 3rd,7th,and 14th day after surgery),the difference is statistically significant(P<0.01).The time of complete incision healing in the research group(28.77±1.43 days)is significantly lower than that in the control group(32.03±2.86 days),the difference is statistically significant(P<0.01).The Wexner score of the research group is lower than that of the control group on the 7th day and 6 months after operation,and the difference is statistically significant(P<0.01).There is no significant difference in postoperative recurrence rate between the two groups(P>0.05).Conclusion The application of anal fistula resection and hanging in the treatment of high anal fistula can significantly reduce postoperative pain,shorten incision healing time,and protect the anal function of patients.
