摘  要:目的本研究旨在评估左室压力-应变环在脓毒症休克患者中的临床意义。方法筛选2022年9月至2023年12月漳州市医院ICU收治的114例脓毒症患者,依据病情严重程度分为脓毒症组和脓毒症休克组,收集两组的临床数据,包括基础信息、实验室检验、心功能指标等。通过多因素Logistic回归,确定与脓毒症休克密切相关的独立危险因素,并利用ROC曲线分析各指标的诊断价值。结果本研究显示,心肌功能障碍、整体做功指数(GWI)<1222 mmHg%、乳酸水平≥3.33 mmol/L、平均动脉压(MAP)<67 mmHg为脓毒症休克的重要预测因素。结论识别这些关键指标可为临床早期干预提供依据,有助于提升救治成功率。Objective This study aimed to evaluate the clinical significance of the left ventricular pressure-strain loop in patients with septic shock.Methods A total of 114 sepsis patients admitted to the ICU of Zhangzhou Hospital between September 2022 and December 2023 were included in the study.Based on disease severity,patients were divided into a sepsis group and a septic shock group.Clinical data,including baseline characteristics,laboratory tests,and cardiac function indicators,were collected.Multivariate Logistic regression was used to identify independent risk factors closely associated with septic shock,and the diagnostic value of these parameters was analyzed using receiver operating characteristic(ROC)curves.Results The study revealed that myocardial dysfunction,global work index(GWI)<1222 mmHg%,lactate concentration≥3.33 mmol/L,and mean arterial pressure(MAP)<67 mmHg were important predictive factors for septic shock.Conclusions Identification of these key indicators may provide a basis for early clinical intervention and improve the success rate of treatment in patients with septic shock.
