安阳地区498例银屑病患者临床资料回顾性分析

摘　　要：目的调查了解安阳地区(五县四区)银屑病的发病情况、临床特点,为该区域的银屑病临床诊断治疗提供数据支撑和循证依据。方法本研究依托国家银屑病规范化诊疗中心临床大数据采集平台,采集的数据来自安阳地区两家大型综合三甲医院皮肤科(患者均为安阳所属县区患者),总样本量498例。本研究回顾性分析了安阳地区银屑病患者一般情况、银屑病临床特点、银屑病伴发疾病、治疗状况、治疗满意度调查。结果本研究共纳入男性患者326例,女性患者172例,男女比例1.90∶1,年龄5~89岁。男女患者的年龄分布规律一致。临床资料特点以寻常型银屑病占比最高,男性患者中寻常型308例(94.48%)、关节病型16例(4.91%)、红皮病型1例(0.31%)、脓疱型1例(0.31%),女性患者中寻常型164例(95.35%)、关节病型7例(4.07%)、红皮病型1例(0.58%)、脓疱型0例。皮损初发于头皮部位的有212例(42.57%),面颈部31例(6.22%),胸背部65例(13.05%),四肢部位198例(39.76%),手足32例(6.43%),外阴3例(0.60%),指甲2例(0.40%)。从银屑病皮损面积和严重程度指数(PASI)评分来看,均值随年龄增长逐渐增高。186例(37.35%)有伴发疾病,内分泌代谢疾病偏多,近1年内采用过外用糖皮质激素治疗的有487例(97.79%)。曾使用或正在使用生物制剂治疗的有35例(7.03%)。传统药物治疗总满意度40%,生物制剂总满意度85%。结论银屑病患者男性多于女性,4种临床类型中男女患病率一致,寻常型银屑病占绝大多数,发病受季节因素影响明显,银屑病共病不容忽视,多数患者仍以使用传统治疗方法为主,疗效满意度不高,使用生物制剂治疗疗效满意度高,但是使用比例较低。Objective To investigate the incidence and clinical characteristics of psoriasis in Anyang area(five counties and four districts),so as to provide data support and evidence-based basis for clinical diagnosis and treatment of psoriasis in this area.Methods This study relied on the clinical big data collection platform of the National Standardized psoriasis Diagnosis and Treatment Center,and collected data from the dermatology department of 2 large general GradeⅢhospitals in Anyang area(patients were all patients in Anyang county),with a total sample size of 498 cases.This study retrospectively analyzed the general situation of psoriasis patients,psoriasis clinical characteristics,psoriasis concomitant diseases,treatment status,treatment satisfaction survey in Anyang area.Results A total of 326 male patients and 172 female patients were included in this study,with a male to female ratio of 1.90:1,aged from 5 to 89 years old.The age distribution of male and female patients was consistent.Psoriasis vulgaris accounted for the highest proportion of clinical data,with 308 cases of vulgaris(94.48%),16 cases of joint disease(4.91%),1 case of erythroderma(0.31%)and 1 case of pustular type(0.31%)in male patients.Among the female patients,there were 164 cases(95.35%)of vulgaris,7 cases(4.07%)of joint disease,1 case(0.58%)of erythroderma and 0 cases of pustular disease.The lesions initially occurred on scalp in 212 cases(42.57%),face and neck in 31 cases(6.22%),chest and back in 65 cases(13.05%),limbs in 198 cases(39.76%),hands and feet in 32 cases(6.43%),vulva in 3 cases(0.60%),and nails in 2 cases(0.40%).According to psoriasis lesion area and severity index scores,the mean PASI increased with age.186 cases(37.35%)had concomitant diseases,endocrine and metabolic diseases were more common,and 487 cases(97.79%)had been treated with external glucocorticoid in the past 1 year.35 cases(7.03%)had been or were being treated with biologics.The total satisfaction of traditional drug treatment was 40%,and the total satisfaction of b
