高频电波刀手术与冷刀宫颈锥切术治疗CINⅢ级的短期并发症比较

摘　　要：目的本研究旨在比较高频电波刀、冷刀宫颈锥切术治疗宫颈上皮内瘤病变(CIN)Ⅲ级的作用。方法本研究纳入2022年5月至2024年5月新疆维吾尔自治区人民医院收治的CINⅢ级患者作为受试者,共128例,按照手术方式的不同分为试验组(高频电波刀手术干预,64例)与对照组(冷刀宫颈锥切术干预,64例),对比两组患者围手术期指标、临床治疗效果、炎性指标及并发症。结果两组治疗总有效率比较无统计学意义(P>0.05)。试验组术中出血量、手术时长、切口愈合时间、炎性指标(IL-8、TNF-α、CRP)、并发症发生率低均低于对照组(P<0.05)。结论高频电波刀手术治疗CINⅢ级患者,手术时长较短,术中出血量偏少,具有较高的临床治疗有效率,可以改善炎性反应,降低并发症发生率,具有较高的临床应用价值。Objective The purpose of this study was to compare the effect of high-frequency wave knife surgery and cold-knife coning of cervix in the treatment of cervical intraepithelial neoplasia(CIN)gradeⅢ.Methods In this study,a total of 128 CINⅢpatients admitted to the People’s Hospital of Xinjiang Uygur Autonomous Region from May 2022 to May 2024 were randomly included as subjects,and were divided into the trial group(high-frequency wave knife surgical intervention,64 cases)and the control group(cold-knife coning of cervix,64 cases)according to different surgical methods.The perioperative indexes,clinical therapeutic effect,inflammatory indexes and complications were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups(P>0.05).The amount of blood loss,operation duration,incision healing time,inflammatory index(IL-8,TNF-α,CRP)and complication rate in trial group were lower than those in control group(P<0.05).Conclusions The treatment of CINⅢgrade patients with high-frequency wave knife surgery has a short operation duration and less intraoperative blood loss,which has high clinical therapeutic effectiveness,can improve inflammatory response and reduce the probability of complications,and has high clinical application value.
