欣母沛联合Bakri球囊填塞与补丁缝合联合术对中央性前置胎盘产后出血的治疗价值研究

摘　　要：目的在应用欣母沛治疗中央性前置胎盘产后出血的基础上实施Bakri球囊填塞与补丁缝合联合术干预,旨在探究药物结合手术治疗的效果和价值。方法根据纳入、排除标准选择莆田市第一医院2020年10月至2022年10月的92例中央性前置胎盘产后出血患者作为研究对象,遵循数表法将其分为对照组和观察组,每组46例。应用欣母沛治疗对照组中央性前置胎盘产后出血,观察组在此基础上联合Bakri球囊填塞与补丁缝合联合术治疗。比较两组临床出血相关指标(止血时间、产后24 h出血量、输血量)、血栓弹力图指标水平。结果经统计分析治疗后的指标水平,显示观察组平均止血时间比对照组短(P<0.05),观察组产后24 h出血量平均值及输血量平均值均比对照组少(P<0.05)。经治疗后,观察组的K凝血时间、反应时间、最大振幅、Angle角均大于对照组(P<0.05)。结论在应用欣母沛治疗中央性前置胎盘的基础上联合Bakri球囊填塞与补丁缝合联合术进行治疗可以改善血栓弹力图水平,加快有效止血速度,减少出血量,降低输血量。Objective To explore the effect and value of drug combined with surgery in the treatment of postpartum hemorrhage caused by central placenta previa with hemabate and Bakri balloon tamponade combined with patch suture.Methods According to the inclusion and exclusion criteria,92 patients with postpartum hemorrhage due to central placenta previa in the Putian First Hospital from October 2020 to October 2022 were selected as the research subjects.They were divided into a control group and an observation group by following the number table method,with 46 cases in each group.Carboprost was applied to treat postpartum hemorrhage due to central placenta previa in the control group,while on this basis,the observation group was treated with the combination of Bakri balloon tamponade and patch suture.The clinical bleeding-related indicators(hemostasis time,the amount of bleeding 24 hours after childbirth,and blood transfusion volume)and the levels of thromboelastography indicators were compared between the two groups.Results After treatment,statistical analysis of the indicator levels,it was shown that the average hemostasis time in the observation group was shorter than that in the control group(P<0.05).The average amount of bleeding 24 hours after childbirth and the average blood transfusion volume in the observation group were both less than those in the control group(P<0.05).After treatment,the coagulation time,reaction time,maximum amplitude,and Angle in the observation group were all greater than those in the control group(P<0.05).Conclusions On the basis of applying carboprost to treat central placenta previa,combined treatment with Bakri balloon tamponade and patch suture can improve the levels of thromboelastography,accelerate the speed of effective hemostasis,reduce the amount of bleeding,and decrease the blood transfusion volume.
