活血化瘀类中成药临床合理使用分析

摘　　要：目的对活血化瘀类中成药在临床应用情况加以分析,同时发现问题,并提出对应的解决办法。方法回顾性分析2021年8月至2023年8月高青县青城卫生院开具的700张活血化瘀类中成药处方单,对其处方单的合理性加以评价,分析不合理原因,并总结患者用药不良反应发生情况。结果700张处方单中,有70张处方单不符合规定用药适应证,占比10%,且70张不合理处方中,其不合理原因主要表现在未辨证用药、配伍禁忌与用药疗程、用药用量不符等。同时,患者在应用不合理活血化瘀类中成药处方单后,其出现的不良反应情况多累及皮肤及其附件、消化与呼吸、神经等系统,且临床多表现为皮肤红疹、恶心呕吐、呼吸急促、晕头、胸闷等。结论活血化瘀类中成药在临床应用上仍存在一些问题,从而导致患者出现不同程度的不良反应,故在实际应用中,需具体结合患者情况、药物配伍禁忌等各方面情况,开具适宜的中成药处方单,从而在保证临床治疗效果的同时,减少患者用药不良反应。Objective To analyze the clinical application of Chinese patent medicines of promoting blood circulation and removing blood stasis,find out the problems and propose corresponding solutions.Methods Retrospective analysis was made on 700 prescriptions of Chinese patent medicines for promoting blood circulation and removing blood stasis issued by Qingcheng Health Center of Gaoqing County from August 2021 to August 2023.The rationality of the prescriptions was evaluated,the unreasonable reasons were analyzed,and the adverse drug reactions of patients were summarized.Results Among 700 prescription forms,70 prescription forms did not meet the prescribed medication indications,accounting for 10%,and 70 unreasonable prescriptions were found.The main reasons for their irrationality were non dialectical medication,compatibility contraindications and medication duration,and unequal medication dosage.At the same time,after the irrational prescription of Chinese patent medicines for promoting blood circulation and removing blood stasis was used,the adverse reactions of patients mostly involved the skin and its accessories,digestive and respiratory systems,nervous systems,and the clinical manifestations were skin rashes,nausea and vomiting,shortness of breath,dizziness,chest tightness,etc.Conclusions There are still some problems in the clinical application of Chinese patent medicines for promoting blood circulation and removing blood stasis,which leads to adverse reactions of different degrees in patients.Therefore,in practical application,it is necessary to issue appropriate prescription of Chinese patent medicines in combination with patients’conditions,drug compatibility taboos and other aspects,so as to ensure the clinical treatment effect and reduce the adverse reactions of patients.
