摘 要:激素性股骨头坏死(SONFH)是一种由长期使用激素类药物引起的难治性骨代谢疾病,发病率、隐匿性和致残率高,严重影响患者日常生活质量和劳作能力,然而其发病机制复杂且尚未完全明了。研究发现,激素通过影响成骨细胞、破骨细胞和骨细胞功能干扰骨代谢平衡;同时,激素也能影响血管内皮细胞的正常功能,导致血管生成障碍,从而加重骨坏死。此外,激素对免疫系统的调节作用也可能在SONFH发病过程中起到重要作用。现对SONFH发病机制研究的主要观点和发现进行总结,以期为临床预防和治疗SONFH提供理论支持和科学依据。Steroid-induced osteonecrosis of the femoral head(SONFH)is a refractory bone metabolism disease caused by long-term use of hormone drugs,which has a high incidence,high insidiousness and high disability rate,and seriously affects the quality of daily life and work ability of patients,but its pathogenesis is complex and not fully understood.Studies have found that hormones interfere with bone metabolic balance by affecting the function of osteoblasts,osteoclasts,and osteocytes.At the same time,hormones can also affect the normal function of vascular endothelial cells,leading to angiogenesis disorders,thereby aggravating osteonecrosis.In addition,hormonal modulation of the immune system may also play an important role in the pathogenesis of SONFH.This article summarizes the main views and findings of the pathogenesis of SONFH,in order to provide theoretical support and scientific basis for clinical prevention and treatment of SONFH.
