针刺联合亚低温对新生儿缺氧缺血性脑病NSE、S-100及aEEG影响观察

摘　　要：目的 观察针刺百会、足三里穴联合亚低温对新生儿缺氧缺血性脑病(HIE)的神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S-100)及振幅整合脑电图(aEEG)的影响,为HIE治疗提供参考。方法 选择2022年1月—2024年6月本院收治的56例HIE新生儿作为研究对象,随机分为对照组和观察组两组,每组各28例。对照组患儿给予亚低温治疗72 h及呼吸、循环、营养支持等治疗;观察组患儿在此基础上予针刺百会、足三里穴,1次/d,连续7 d。两组患儿入院时进行NSE、S-100、aEEG检查,亚低温治疗72 h结束后复查aEEG,治疗7 d后复查NSE、S-100。记录患儿一般情况、住院天数及预后等信息。结果 入院当天对照组、观察组患儿NSE分别为(33.76±16.82)ng/ml、(30.76±21.52)ng/ml,差异无统计学意义(P>0.05);入院当天入组后对照组、观察组患儿S-100分别为(9.15±15.06)ng/ml、(5.62±11.21)ng/ml,差异无统计学意义(P>0.05);入院当天对照组患儿检查aEEG轻度异常、中度异常、重度异常患儿例数分别为1例、15例、12例,观察组患儿aEEG轻度异常、中度异常、重度异常患儿例数分别为2例、12例、14例,差异无统计学意义(P>0.05)。治疗3 d,对照组26例患儿复查aEEG,结果aEEG正常、轻度异常、中度异常、重度异常患儿例数分别为8例、14例、3例、1例,观察组25例患儿复查aEEG,结果aEEG正常、轻度异常、中度异常、重度异常患儿例数分别13例、10例、2例、0例,差异具有统计学意义(P>0.05)。治疗7 d后,复查NSE在对照组(21例)、观察组(22例)分别为(11.71±4.59)ng/ml、(9.18±3.43)ng/ml,观察组明显低于对照组患儿NSE水平,差异具有统计学意义(P<0.05);复查S-100在对照组(21例)、观察组(22例)分别为(0.91±0.58)ng/ml、(0.61±0.37)ng/ml,观察组明显低于对照组患儿S-100水平,差异具有统计学意义(P<0.05)。对照组患儿放弃治疗4例,其中2例患儿放弃治疗后死亡;观察组患儿住院期�Objective To observe the effect of acupuncture at Baihui and Zusanli acupoints combined with mild hypothermia in neonates with hypoxic-ischemic encephalopathy(HIE)on neuron-specific enolase(NSE),central nervous system-specific protein(S-100),and amplitude-integrated electroencephalography(aEEG),providing reference for the treatment of HIE.Methods A total of 56 neonates with HIE admitted to our hospital from January 2022 to June 2024 were selected as subjects and randomly divided into a control group and an observation group,with 28 cases in each group.The control group received mild hypothermia for 72 hours,along with respiratory,circulatory,and nutritional support.The observation group received additional acupuncture at Baihui and Zusanli acupoints once daily for 7 consecutive days.NSE,S-100,and aEEG were examined at admission.aEEG was re-examined after 72 hours of mild hypothermia,and NSE and S-100 were re-examined after 7 days of treatment.General condition,hospitalization days,and prognosis were recorded.Results At admission,the NSE levels in the control and observation groups were(33.76±16.82)ng/ml and(30.76±21.52)ng/ml,respectively,with no significant difference between the groups(P>0.05).The S-100 protein levels were(9.15±15.06)ng/ml in the control group and(5.62±11.21)ng/ml in the observation group,with no significant difference(P>0.05).The number of mild,moderate,and severe aEEG abnormalities in the control group was 1,15,and 12,respectively,and in the observation group was 2,12,and 14,respectively,with no significant difference(P>0.05).After three days of treatment,the number of normal aEEG and mild,moderate,and severe aEEG abnormalities in the control group was 8,14,3,and 1,respectively(26 cases re-examined),and in the observation group was 13,10,2,and 0,respectively(25 cases re-examined),with a significant difference(P<0.05).After seven days of treatment,the NSE levels in the control group(21 cases re-examined)and observation group(22 cases re-examined)were(11.71±4.59)ng/ml and(9.18±3.43)ng/ml,r
