经桡动脉与经股动脉两种途径在肝细胞癌患者接受肝动脉灌注化疗的舒适度比较

摘　　要：目的 评估肝动脉灌注化疗(HAIC)采用不同途径时对患者舒适度、安全性及日常生活能力的影响。方法 回顾性分析2022年9月—2023年9月广东省某肿瘤专科医院微创介入科行HAIC治疗的68例肝细胞癌患者的临床资料,以上患者均接受经股动脉和经桡动脉两种途径行穿刺置管手术各1次,进行自身交叉对照。采用舒适状况量表(GCQ)和巴氏指数(Barthel index)评估实施以上两种途径HAIC时患者的舒适度和日常生活能力,同时记录手术相关的副反应。结果 患者经桡动脉与经股动脉两种不同途径在接受HAIC治疗时,舒适度和日常生活能力方面存在显著差异[分别为(86.4±9.16)分vs(81.3±8.84)分,P<0.001];以及[(83.8±8.90)分vs (69.4±13.4)分,P<0.001]。结论 经桡动脉途径行HAIC治疗时患者的舒适度及日常生活能力都明显优于经股动脉途径。经桡动脉途径的安全性和耐受性方面均表现良好,能够在连续的动脉灌注化疗期间显著提高患者的生活质量和舒适度,有利于优化患者的就医体验。Objective To assess the impact of different routes of hepatic arterial infusion chemotherapy(HAIC)on patient comfort,safety,and daily living abilities.Methods Retrospective analysis was conducted on the clinical data of 68 hepatocellular carcinoma patients who underwent HAIC via femoral and radial artery approaches at the Department of Minimally Invasive Intervention,a tumor specialist hospital in Guangdong Province,from September 2022 to September 2023.Each patient underwent puncture catheterization surgery via both femoral and radial artery approaches,serving as their own crossover control.The General Comfort Questionnaire(GCQ)and Barthel Index were used to evaluate the comfort level and activities of daily living for patients undergoing both radial and femoral artery access,with procedure-related complications recorded.Results There were significant differences in comfort and ability to perform activities of daily living between patients with radial artery access and those with femoral artery access(86.4±9.16 vs.81.3±8.84,P<0.001;83.8±8.90 vs.69.4±13.4,P<0.001).Conclusions The comfort and daily living ability of patients undergoing HAIC via transradial artery access were significantly better than those undergoing treatment via transfemoral artery access.The safety and tolerance of transradial artery access were good,significantly improving the quality of life and comfort of patients during continuous arterial infusion chemotherapy and optimizing the patient's medical experience.
