摘　　要：妊娠晚期孕妇因胎儿增长较大,多伴随一些生理性的改变,如有压迫感、呼吸比较粗、呼吸困难、心率比较快、心悸气短、下腹部和腰部有疼痛、下肢有水肿、小便次数多、睡眠时不能平躺等,这些都是正常症状,在分娩后就会得到缓解。而这一特殊时期若合并有心血管疾病,也会出现病理性的改变,造成高危妊娠。体外膜氧合是代表一个医院,甚至一个地区、一个国家危重症急救水平的一门技术,是围产期心肺衰竭患者的重要治疗手段。本文报道1例房间隔缺损封堵术后合并肺动脉高压的孕晚期发生心脏骤停患者采用体外膜氧合二级转运联合主动脉内球囊反搏救治成功的过程,以期为临床妊娠合并心血管疾病重症患者的诊治提供参考。Pregnant women in late pregnancy often experience physiological changes due to significant fetal growth,such as pressure,rough breathing,difficulty breathing,fast heart rate,palpitations,shortness of breath,pain in the lower abdomen and waist,edema in the lower limbs,frequent urination,and inability to lie flat during sleep.These symptoms are normal and will be relieved after delivery.If cardiovascular disease is present during this special period,pathological changes may also occur,leading to high-risk pregnancies.Extracorporeal membrane oxygenation(ECMO)is a technique that represents the level of critical care in a hospital,even a region or a country,and is an important treatment for patients with perinatal heart and lung failure.This article reports the successful treatment of a late pregnancy cardiac arrest patient with pulmonary arterial hypertension after atrial septal defect occlusion surgery using ECMO secondary transport combined with intra-aortic balloon pump.The aim is to provide reference for the diagnosis and treatment of severe pregnancy related cardiovascular disease patients in clinical practice.
