关节镜下松解髂胫束治疗外侧型弹响髋的临床疗效分析

摘　　要：目的 评估关节镜下松解髂胫束治疗外侧型弹响髋的临床效果。方法 2019年7月—2021年2月,回顾性分析上海市某医院骨科对保守治疗至少6个月且无效的外侧型弹响髋并进行了关节镜手术治疗的临床资料,通过比较术前和术后12个月患者临床症状,以及髋关节内收角度、改良髋关节Harris评分(mHHS)、髋关节功能评分—日常生活量表(HOS-ADL)、疼痛视觉模拟评分(VAS)评估临床疗效。结果 共纳入42例患者(男17例,女25例),年龄19~42岁,平均(31.2±5.20)岁,随访时间13~29个月,平均(19.7±3.84)个月。所有患者弹响完全消失Ober征阴性,且均无感染、血肿、神经损伤等手术并发症。术后12个月随访时髋关节内收角度从术前-32.3±10.03提高到20.6±2.91,mHHS评分从术前(65.8±5.52)分上升至(92.6±2.38)分,HOS-ADL从术前(70.1±5.32)分提高(93.5±2.58)分,VAS评分由术前(5.9±1.31)分降至(0.4±0.67)分,差异均有统计学意义(P<0.05)。结论 关节镜下松解髂胫束治疗外侧型弹响髋具有创伤小,恢复快,且无严重并发症等优点,其疗效明确预后较好,是一种安全有效的技术。Objective To evaluate the clinical effect of arthroscopic release of iliotibial band in the treatment of external snapping hip.Methods From July 2019 to February 2021,patients with external snapping hip who had undergone at least 6 months of conservative treatment without success were selected from the Department of Orthopedics at Shanghai Tenth People's Hospital for arthroscopic surgery.The clinical outcomes were assessed by comparing preoperative and 12-month postoperative clinical symptoms,hip adduction angle,modified Harris Hip Score(mHHS),Hip Outcome Score-Activities of Daily Living(HOS-ADL),and Visual Analogue Scale(VAS)for pain.Results A total of 42 patients(17 males and 25 females),aged 19 to 42 years(mean age 31.2±5.20 years),were followed up for 13 to 29 months(mean follow-up 19.7±3.84 months).All patients experienced complete resolution of snapping and a negative Ober test,with no surgical complications such as infection,hematoma,or nerve injury.At the 12-month postoperative follow-up,the hip adduction angle increased from-32.3±10.03 to 20.6±2.91;the mHHS score improved from 65.8±5.52 to 92.6±2.38;the HOS-ADL score increased from 70.1±5.32 to 93.5±2.58;the VAS score decreased from 5.9±1.31 to 0.4±0.67;all differences were statistically significant(P<0.05).Conclusions Arthroscopic release of the iliotibial band for the treatment of external snapping hip is associated with minimal trauma,rapid recovery,and no severe complications.It offers clear therapeutic benefits and favorable outcomes,making it a safe and effective technique.
