摘 要:目的 探讨3D打印技术在锁骨骨折内固定术中的应用价值。方法 选择2022年10月—2023年10月本院收治的锁骨骨折患者30例作为研究对象,根据患者是否使用3D打印技术分为对照组和研究组两组,每组各15例。对照组采用常规手术方法治疗锁骨骨折。研究组通过3D打印技术打印出复位的锁骨模型,进行体外模拟实验,选择合适的钢板和螺钉,预弯钢板后进行手术。比较两组患者手术时间、术中出血量、透视次数、螺钉更换次数、切口长度和并发症发生率,并于术后3个月采用Constant-Murley肩关节评分、Neer评分进行临床疗效评价。结果 两组患者均顺利完成手术,术后未出现切口感染、神经损伤、骨愈合不良等并发症。研究组的手术时间(67.87±21.06)min、术中失血量(53.33±19.52)ml、螺钉更换次数(1.40±1.06)次和术中透视次数(2.22±0.77)次、切口长度(8.20±1.56)cm明显少于对照组(86.93±14.38)min、(76.00±17.65)ml、(2.27±1.22)次、(3.73±0.88)次、(10.20±1.78)cm,差异均有统计学意义(P<0.05)。两组患者术后3个月的Constant评分、Neer评分比较,差异无统计学意义(P>0.05)。结论 3D打印技术辅助锁骨骨折内固定治疗是一种安全、有效、简便的方法,与常规手术方法相比,可有效减少术中失血,缩短手术时间,减少术中透视次数,值得临床进一步地研究和使用。Objective To investigate the application value of 3D printing technology in internal fixation of clavicular fractures.Methods Thirty patients with clavicle fractures treated in our department from October 2022 to October 2023 were selected as study subjects and divided into two groups based on the use of 3D printing technology:a control group and a study group,each consisting of 15 cases.The control group was treated with conventional surgery for clavicle fractures.The study group used 3D printing technology to print a reduced clavicle model for in vitro simulation experiments,select appropriate plates and screws,and perform surgery after pre-bending the plates.Operative time,intraoperative blood loss,fluoroscopy times,screw replacement times,incision length,and complication rate were compared between the two groups.Clinical efficacy was evaluated using the Constant-Murley shoulder joint score and Neer score three months postoperatively.Results Both groups successfully completed the operation without postoperative complications such as wound infection,nerve injury,or poor bone healing.The study group had significantly reduced operative time(67.87±21.06 minutes),intraoperative blood loss(53.33±19.52 ml),screw replacement times(1.40±1.06),intraoperative fluoroscopy times(2.22±0.77),and incision length(8.20±1.56 cm)compared to the control group(86.93±14.38 minutes,76.00±17.65 ml,2.27±1.22 times,3.73±0.88 times,10.20±1.78 cm),with statistically significant differences(P<0.05).There was no statistical significance in the Constant and Neer scores at three months postoperatively between the two groups(P>0.05).Conclusions 3D printing technology-assisted internal fixation treatment of clavicle fractures is a safe,effective,and simple method.Compared to conventional surgical methods,it can effectively reduce intraoperative blood loss,shorten operative time,and decrease intraoperative fluoroscopy times,making it worthy of further clinical research and use.
