温阳化痰方治疗老年缺血性脑卒中疗效及相关机制

摘　　要：目的:探讨温阳化痰方治疗老年缺血性脑卒中(IS)临床疗效及其治疗机制。方法:选择老年IS患者110例,按照随机数字表法分为观察组、对照组,每组55例。对照组予以规范的西医治疗,观察组在此基础上口服中药温阳化痰方。比较两组治疗前后中医证候评分、美国国立卫生研究院卒中量表(NIHSS)评分、Berg平衡量表(BBS)评分、Sheikh控制量表(SCS)评分以及患者血清多巴胺(DA)、乙酰胆碱(ACh)、神经调节蛋白1(NRG1)、早期生长反应因子-1(Egr-1)、音猬因子(SHH)水平、长链非编码RNA小核仁RNA宿主基因8(LncRNA SNHG8)相对表达量。比较两组治疗效果。结果:治疗后,观察组各项中医证候评分、NIHSS评分低于对照组,BBS、SCS评分高于对照组,差异有统计学意义(均P<0.05)。治疗后,观察组血清DA、ACh、NRG1、SHH水平及LncRNA SNHG8相对表达量高于对照组,血清Egr-1水平低于对照组,差异有统计学意义(均P<0.05)。治疗总有效率高于对照组(P<0.05)。结论:温阳化痰方治疗老年IS效果较好,其机制可能与调节SHH、LncRNA SNHG8有关。Objective:To explore clinical efficacy of Wenyang Huatan formula in treatment of elderly ischemic stroke(IS)and analyze its treatment mechanism preliminarily.Methods:110 elderly IS patients were selected and divided into observation group and control group according to digital random table method,with 55 cases in each group.The control group received standardized western medicine treatment,while the observation group took Wenyang Huatan formula orally on the basis of the control group.The traditional Chinese medicine syndrome scores,NIHSS scores,BBS scores and SCS scores before and after treatment between the two groups were compared,serum dopamine(DA),acetylcholine(ACh),neuroregulatory protein 1(NRG1),Egr-1,levels of sound hedgehog factor(SHH)and relative expression level of long chain non coding RNA small nucleolar RNA host gene 8(LncRNA SNHG8)were compared.The total efficacy of the two groups were compared.Results:After treatment,scores of various traditional Chinese medicine symptoms and NIHSS of the observation group were lower than those in the control group,while the scores of BBS and SCS were higher than those in the control group(all P<0.05).The levels of serum DA,ACh,NRG1,SHH and relative expression of LncRNA SNHG8 of the observation group were higher than those in the control group,while the levels of serum Egr-1 were lower than those of the control group(all P<0.05).The total effective rate of treatment was higher than that of the control group(P<0.05).Conclusion:Wenyang Huatan has significant therapeutic effect on elderly IS.Its mechanism may be related to regulation of SHH and LncRNA SNHG8.
