摘　　要：目的探索依据“筋骨差爻”理论精准定位病灶,合并“三维定点一次疗法”复位半脱位的关节突关节在脊柱退行性变正骨疗法中的疗效及其理论依据。方法在腰椎间盘突出、非特异性腰背痛和颈椎病的正骨治疗中,应用“筋骨差爻”理论精准定位,通过“三维定点一次疗法”复位异常关节,并观察疼痛、活动受限改善情况,进行影像学检查,观察改变。结果3例患者在治疗后关节疼痛和活动受限的情况均有好转,影像学检查提示2例颈椎病患者的颈椎曲度有恢复。非特异性腰背痛患者在疼痛位置观察到胸椎曲度变化、椎体唇样变和椎间孔变小,治疗后曲度和椎体唇样变无明显改变,椎间孔略有恢复。结论脊柱退行性变可能与关节突关节半脱位有关,且针对半脱位关节进行正骨治疗可取得良好疗效,“筋骨差爻”理论具有临床适应性。Objective To investigate the effectiveness and theoretical foundation of precisely locating lesions using the"sinew and bone dislocation"theory,combined with"three-dimensional fixed-point one-time therapy",in the realignment of subluxated zygapophyseal joints during the treatment of spinal degeneration with the bone-sitting therapy.Methods During the traditional Chinese treatment of lumbar disc herniation,nonspecific low back pain,and cervical spondylosis,the"sinew and bone dislocation"theory was utilized to precisely locate and realign abnormal joints through the"three-dimensional fixed-point one-time therapy".The relief in pain and improvement in limited range of motion were observed,and imaging examinations were conducted to assess changes.Results After treatment,alleviations of joint pain and limited range of motion were observed in all three patients.Imaging findings showed the reverse of the cervical curvature in two patients with cervical spondylosis.In patients with nonspecific low back pain,changes in thoracic curvature,lip-like changes in vertebral bodies,and smaller intervertebral foramen were observed at the site of pain.Within the following treatment,pain at site relieved,while the size of intervertebral foramen slightly increased,but there was no significant change in curvature and lip-like changes in vertebral bodies.Conclusion Spinal degenerative changes may be associated with zygapophyseal subluxation,and bone-sitting treatment targeting the subluxated joints can yield favorable therapeutic outcomes.The"sinew and bone dislocation"theory demonstrates clinical applicability.
