早孕期胸腹联体双胎的超声图像特征分析

摘　　要：目的总结早孕期胸腹联体双胎的超声图像特征。方法回顾性分析我院经引产证实的7例孕8~13周胸腹联体双胎的超声图像特征,并与引产后大体观进行对照分析。结果7例早孕期胸腹联体双胎矢状切面均可见头尾呈“分叉”征象且无法获得标准正中矢状切面,胎儿横切面见胸腹壁连接处体表均存在切迹,呈“∞”征。胸腹联体双胎均仅见1个节律的心脏跳动且位置居中。7例均选择引产,引产大体观与产前超声诊断相符。结论早孕期胸腹联体双胎的典型超声图像特征为胎儿矢状切面见头尾呈“分叉”征象,横切面见胸腹壁连接处体表切迹呈“∞”征,且仅可见1个节律的心脏跳动。Objective To summarize the ultrasound imaging features of thoracopagus conjoined twins in the frist pregnancy.Methods A retrospective analysis was conducted on ultrasound images of 7 cases of thoracopagus conjoined twins at 8~13 gestational weeks confirmed by induced labor in our hospital.The ultrasound findings were compared with post-mortem observations following labor induction.Results In the first pregnancy,the sagittal section of thoracopagus conjoined twins revealed a characteristic“forking”pattern,precluding the acquisition of a standard sagittal view.In the transverse section of the fetal chest and abdomen,distinct notches were observed at the site of fetal body fusion,presenting an“infinity”(∞)sign.Additionally,all thoracopagus conjoined twins exhibited a single,centrally located rhythmic heartbeat.All 7 cases of conjoined twins opted for induced labor,and the post-mortem appearance was consistent with the prenatal ultrasound diagnosis.Conclusion The defining ultrasound imaging features of thoracopagus conjoined twins in the first trimester include a“forking”phenomenon observed in the sagittal view,an“infinity”(∞)sign in the transverse view at the fetal body connection site,and the visualization of a single,rhythmic heartbeat.
