摘 要:目的应用剪切波弹性成像(SWE)定量评估特定体位下正常青年人踝关节不同角度时小腿三头肌硬度变化。方法选取正常青年志愿者50例,其中男、女各25例,于俯卧位膝关节完全伸展体位下改变踝关节角度为背伸10°、中立位0°、跖屈30°和跖屈50°时,应用SWE分别测量双侧小腿三头肌(腓肠肌内侧头、腓肠肌外侧头、比目鱼肌)剪切波速度(SWV),比较踝关节不同角度时,以及不同性别、惯用侧与非惯用侧踝关节同一角度时小腿三头肌SWV的差异。结果腓肠肌内、外侧头及比目鱼肌踝关节不同角度时SWV比较差异均有统计学意义(均P<0.001),其中背伸10°时腓肠肌内、外侧头SWV均大于中立位0°、跖屈30°、跖屈50°时SWV,跖屈30°时腓肠肌内侧头SWV小于其他角度时SWV,跖屈50°时比目鱼肌SWV大于跖屈30°、中立位0°时SWV,背伸10°时比目鱼肌SWV大于跖屈30°、中立位0°时SWV,差异均有统计学意义(均P<0.05);中立位0°、背伸10°时腓肠肌内侧头SWV均大于腓肠肌外侧头SWV,跖屈30°、跖屈50°时比目鱼肌SWV均大于腓肠肌内、外侧头SWV,中立位0°、背伸10°时比目鱼肌SWV均小于腓肠肌内、外侧头SWV,差异均有统计学意义(均P<0.05)。男、女性小腿三头肌SWV随踝关节角度改变的变化趋势相同,且男性踝关节中立位0°、背伸10°时腓肠肌内、外侧头SWV均大于女性,差异均有统计学意义(均P<0.05)。惯用侧与非惯用踝关节同一角度时小腿三头肌SWV比较差异均无统计学意义。结论SWE可用于定量评估正常青年人踝关节不同角度时小腿三头肌硬度变化。Objective To quantitatively evaluate the stiffness changes of the triceps surae muscle at different ankle angles in normal young people under particular positions by shear wave elastography(SWE).Methods Totally 50 young volunteers(25 males and 25 females)were selected.When the ankle angle was changed to dorsal 10°,neutral 0°,plantarflexion 30°and plantarflexion 50°under the prone position with the knee fully extended,the shear wave velocity(SWV)of bilateral triceps surae muscle(medial head of gastrocnemius,lateral head of gastrocnemius and soleus muscle)were measured by SWE.The differences of SWV of triceps surae muscle between different ankle angles,different genders,and the dominant and nondominant sides were compared.Results The SWV at different ankle joint angles in the medial and lateral heads of the gastrocnemius and the soleus muscle were statistically significant differences(all P<0.001).The SWV of the medial and lateral heads of the gastrocnemius at dorsal extension 10°were higher than those at neutral 0°,plantarflexion 30°and plantarflexion 50°,and the SWV of the medial head of gastrocnemius at plantarflexion 30°was lower than that at other angles,and the SWV of soleus muscle at plantarflexion 50°was higher than that at neutral 0°,plantarflexion 30°,and the SWV of soleus muscle at dorsal extension 10°was higher than those at plantarflexion 30°and neutral 0°,with statistically significant differences(all P<0.05).The SWV of the medial head of the gastrocnemius muscle at neutral 0°and dorsal extension 10°were higher than those of the lateral head of the gastrocnemius muscle,and the SWV of soleus muscle at plantarflexion 30°and plantarflexion 50°were higher than those of the medial and lateral heads of gastrocnemius,and the SWV of soleus muscle at neutral 0°and dorsal extension 10°were lower than those of the medial and lateral heads of gastrocnemius,with statistically significant differences(all P<0.05).The change trend of SWV of the triceps surae muscle with the change of angle was th
