超声多模态特征诊断乳腺病变的作用分析

摘　　要：目的 探究超声多模态特征诊断乳腺病变的作用。方法 回顾性分析我院2022年3月至2024年3月收治的61例乳腺病变患者的临床资料,均行多模态超声诊断。以病理诊断结果为金标准,对比良性病变与恶性病变的多模态超声征象;绘制受试者工作特征(ROC)图,分析多模态超声诊断乳腺病变的诊断效能。结果 61例患者经病理结果分析,良性病变21例(34%),恶性病变40例(66%)。恶性病变组患者的不规则形状、不平行生长方向、边缘不完整、内部回声不均匀、微钙化、后方回声衰减、病灶周围及内部血流、超声弹性评分4~5分发生率均高于良性病变组,差异有统计学意义(P<0.05)。多模态超声诊断乳腺病变的灵敏度是98%,特异度是71%,准确率是89%,阳性预测值87%,阴性预测值94%,kappa值=0.73。绘制ROC曲线,曲线下面积(95%CI)是0.913(0.824,0.962)。结论 乳腺良性病变与恶性病变的超声多模态特征不同,临床可以此为依据鉴别诊断疾病,且诊断效能佳。Objective To explore the role of ultrasound multimodal features in the diagnosis of breast lesions.Methods The clinical data of 61 patients with breast lesions admitted to our hospital from March 2022 to March 2024 were diagnosed by multimodal ultrasound.Taking pathological diagnostic results as the gold standard,compare the multimodal ultrasound signs of benign lesions and malignant lesions.The receiver operator characteristic curve(ROC)was used to analyze the diagnostic efficacy of multimodal ultrasound for detecting breast lesions.Results In 61 patients,21(34%)and 40(66%)were malignant lesions.In the malignant lesion group,the incidence of irregular shape,unparallel growth direction,incomplete margin,uneven internal echo,microcalcification,posterior echo decay,peripheral and internal blood flow,and ultrasound elasticity score of 4 to 5 were all higher than those in the benign lesion group,and the difference was statistically significant(P<0.05).The sensitivity of multimodal ultrasound to diagnose breast lesions was 98%,specificity was 71%,accuracy rate was 89%,positive predictive value was 87%,negative predictive value was 94%,and kappa value=0.73.ROC curve and the area under the curve(95%CI)was 0.913(0.824,0.962).Conclusion The ultrasound multimodal characteristics of benign breast lesions are different from malignant lesions,which can be based on the differential diagnosis of disease,and the diagnostic efficiency is good.
