摘 要:目的 分析颅脑磁共振成像(MRI)在新生儿窒息后颅脑损伤诊断中的应用价值。方法 回顾性分析2019年3月至2022年3月我院收治并经病理检查确诊的30例新生儿窒息后颅脑损伤患儿的临床资料,均进行颅脑MRI、CT检查,以病理检查为金标准,比较2组各部位损伤诊断准确率。结果 与组织病理学结果对照,MRI诊断颅脑损伤28例,诊断准确率为93%、灵敏度为96%、特异度为86%、阳性预测值为96%,阴性预测值为86%;CT诊断颅脑损伤22例,诊断准确率为73%、灵敏度为86%、特异度为78%、阳性预测值为90%,阴性预测值为70%;MRI诊断颅脑损伤的准确率、灵敏度、特异度、阳性预测值及阴性预测值均高于CT诊断(P<0.05);MRI对脑实质出血、脑室内出血检出率高于CT(P<0.05)。结论 颅脑MRI在新生儿窒息后颅脑损伤诊断中具有一定的应用临床价值,通过MRI检查可确定颅脑损伤区域、数量,能够为治疗提供参考。Objective To analyze the application value of craniocerebral MRI in the diagnosis of craniocerebral injury after neonatal asphyxia.Methods A retrospective analysis was performed on the clinical data of 30 neonates confirmed with craniocerebral injury after asphyxia by pathological examination who were admitted to the hospital from March 2019 to March 2022.MRI and CT were performed in both groups,and pathological examination was used as the gold standard to compare the diagnostic rate of various parts of the two groups.Results Compared with histopathological results,MRI diagnosis showed that there were 28 cases with craniocerebral injury,its diagnostic accuracy rate,sensitivity,specificity,positive predictive value and negative predictive value were 93%,96%,86%,96%and 86%,respectively.CT diagnosis showed that there were 22 cases with craniocerebral injury,its diagnostic accuracy rate,sensitivity,specificity,positive predictive value and negative predictive value were 73%,86%,78%,90%and 70%,respectively.The diagnostic accuracy rate,sensitivity,specificity,positive predictive value and negative predictive value of MRI in the diagnosis of craniocerebral injury were all higher than those of CT(P<0.05).Conclusion Craniocerebral MRI has certain clinical application value in the diagnosis of craniocerebral injury after neonatal asphyxia.The area and number of craniocerebral injury can be determined by MRI examination,which can provide reference for treatment.
