三维超声检查在输卵管间质部妊娠诊断中的应用价值

摘　　要：目的 探讨三维超声(3D-TVS)检查在输卵管间质部妊娠(ITP)诊断中的应用价值。方法选取64例我院2019年9月至2023年9月经二维超声检查疑似ITP患者,所有患者均行3D-TVS检查,以手术病理检查结果为“金标准”,比较2种诊断方式的检查诊断结果、诊断效能及对不同类型ITP检出率,分析ITP超声检查表现。结果 64例疑似ITP患者中,12例宫角妊娠,52例输卵管间质妊娠;经二维超声检查,宫角妊娠26例,输卵管间质妊娠38例;经3D-TVS检查,宫角妊娠17例,输卵管间质妊娠47例;3D-TVS检查灵敏度83%(43/52)、准确率80%(51/64)较二维超声检查62%(32/52)、64%(41/64)高,漏诊率17%(9/52)较二维超声检查38%(20/52)低,差异有统计学意义(P<0.05);与二维超声检查相比,3D-TVS检查对于孕囊型、不典型ITP患者检出率较高(P<0.05)。结论 3D-TVS检查诊断准确率明显高于常规超声检查,可有效提高诊断效能,对临床筛查诊断、制定治疗方案具有重要意义。Objective To explore the application value of 3D-TVS(three-dimensional ultrasound)examination in the diagnosis of tubal interstitial pregnancy(ITP).Methods A total of 64 patients with suspected tubal interstitial pregnancy who underwent a two-dimensional ultrasound examination in our hospital from September 2019 to September 2023 were selected,and all patients underwent 3D-TVS examination.Taking the results of surgical pathology as the"gold standard",the diagnostic results,diagnostic efficacy and detection rate of different types of tubal ITP between the two diagnostic modalities were analyzed.Results Among 64 patients with suspected tubal ITP,there were 12 cases with cornual pregnancy and 52 cases with tubal interstitial pregnancy.26 cases with cornual pregnancy and 38 cases with tubal interstitial pregnancy were found by two-dimensional ultrasound examination.Seventeen cases with cornual pregnancy and 47 cases with tubal interstitial pregnancy were found by 3D-TVS examination.The sensitivity of 3D-TVS was 83%(43/52),and the accuracy[80%(51/64)]was higher than that of 2D ultrasound[62%(32/52),64%(41/64)],and the missed diagnosis rate of 17%(9/52)was lower than that of 2 D ultrasound 38%(20/52),and the difference was significant(P<0.05).Compared with two-dimensional ultrasound examination,the detection rate of 3D-TVS examination for gestational sac type and atypical tubal ITP was higher(P<0.05).Conclusion The diagnostic accuracy of 3D-TVS is significantly higher than that of conventional ultrasound examination,which can effectively improve diagnostic efficiency and is of great significance for clinical screening diagnosis and treatment.
