从“孙络-微血管”理论探究助卵汤联合盆底电刺激治疗肾虚证卵巢功能减退临床疗效

摘　　要：目的观察助卵汤联合盆底电刺激治疗肾虚证卵巢储备功能减退(diminished ovarian reserve,DOR)的临床疗效,探究在“孙络-微血管”理论指导下如何提高患者卵巢储备功能。方法选取2022年12月—2023年12月就诊于辽宁中医药大学附属医院妇产科并确诊为肾虚证DOR的患者60例,在患者自愿的基础上选择治疗方案,分为对照组30例、治疗组30例,两组患者均予芬吗通及助卵汤口服,治疗组在对照组基础上加用盆底生物反馈电刺激治疗方案,两组疗程均为3个月经周期。比较两组患者治疗前后的中医证候积分,检测两组患者血清雌二醇(estradial,E2)、卵泡刺激素(follicle-stimulating hormone,FSH)和抗缪勒管激素(anti-Müllerian hormone,AMH)水平,观察两组患者窦卵泡计数(antral follicle counting,AFC)、卵巢体积(ovarian volume,OV)变化情况,治疗期间记录两组患者不良反应发生情况。结果治疗后,治疗组患者总有效率(90.0%,27/30)显著高于对照组(56.7%,17/30);治疗组患者中医证候积分与对照组相比降低显著,差异有统计学意义(P<0.05);治疗组患者FSH水平显著低于对照组(P<0.01);E2水平显著高于对照组(P<0.01);AFC、OV高于对照组(P<0.05)。结论基于“孙络-微血管”理论,助卵汤联合盆底电刺激能降低患者中医证候积分,显著改善DOR患者的激素水平,提高卵巢功能。Objective To explore the clinical efficacy of Zhuluan Decoction(助卵汤)combined with pelvic floor electrictrotherapy in treating diminished ovarian reserve(DOR)of kidney deficiency syndrome,and to explore how to improve the ovarian reserve function of patients under the guidance of“minute collateral-microvascular”theory.Methods A total of 60 patients diagnosed with DOR of kidney deficiency syndrome were selected from the obstetrics and gynecology department of the affiliated hospital of Liaoning university of traditional Chinese medicine from December 2022 to December 2023.The patients were divided into control group(30 cases)and treatment group(30 cases)on the basis of voluntary treatment.Both groups were given Fenmorton and Zhuluan Decoction orally.The treatment group was added with pelvic floor biofeedback electrical stimulation on the basis of the control group,and the course of treatment in both groups was 3 menstrual cycles.TCM syndrome scores before and after treatment were compared between the two groups,serum estradiol(E2),follicle-stimulating hormone(FSH)and anti-Müllerian hormone(AMH)were detected,changes in antral follicle counting(AFC)and ovarian volume(OV)were observed,and adverse reactions were recorded during treatment.Results After treatment,the total effective rate of treatment group(90.0%,27/30)was significantly higher than that of control group(56.7%,17/30).TCM syndrome scores of the treatment group patients decreased significantly compared with the control group,and the difference was statistically significant(P<0.05).The level of FSH in treatment group was significantly lower than that in control group(P<0.01).The level of E2 in treatment group was significantly higher than that of control group(P<0.01).AFC and OV in treatment group were higher than those of control group(P<0.05).Conclusion Based on the theory of“minute collateral-microvascular”,Zhuluan Decoction combined with pelvic floor electric stimulation can reduce the TCM syndrome score of patients,significantly improve th
