基于“先安未受邪之地”理论探讨中医药对原发肿瘤转移前肺生态位影响

摘　　要：癌症转移是肿瘤患者死亡的重要原因,而肺是常见的转移器官。研究发现,肺转移前生态位的改变为癌细胞提供了有利的“土壤”,如果在转移之前能够改变肺内微环境,使得癌细胞“种子”无法种植,将在阻止癌症的转移中发挥巨大作用。中医经典《温热论》提出“先安未受邪之地”理论,即根据疾病的发展趋势和有可能发生转变的部分,及时有效地采取措施阻止疾病的进一步发展和传变,以预防为主,祛邪与扶正相结合减少肿瘤细胞转移。中医学的四诊合参辨证论治与西医对癌症转移分子机制认识相结合,中医预防性思维,及早对未受邪之地施以药物顾护,西医在分子水平上对各种转移通路及时阻断,从而达到“先安”的效果,这一治病原则对于提高临床疗效必将发挥其积极作用,为临床中西医结合抗肿瘤转移治疗提供思路。Cancer metastasis is an important cause of death in tumor patients,and the lung is a common metastasis organ.Modern medical research has found that changes in the ecological niche before lung metastasis provide favorable“soil”for cancer cells.If the microorganisms in the lungs can be changed before metastasis so that cancer cell“seeds”cannot be planted,it will play a role in preventing cancer metastasis.The theory of“first settling in the uninfected place”put forward by the TCM classic Warming Theory means taking timely and effective measures to prevent the further progression and spread of the disease based on the trend of the disease and the parts that are likely to change,focusing on prevention.,the combination of dispelling pathogenic factors and strengthening the body's vitality can prevent tumor cell metastasis.The four diagnosis methods of traditional Chinese medicine combined with ginseng syndrome differentiation and treatment are combined with Western medicine's understanding of the molecular mechanism of cancer metastasis.Traditional Chinese medicine's preventive thinking provides early drug care to the“uninfected place”,and Western medicine can promptly detect various metastasis pathways at the molecular level.Blocking,so as to achieve the effect of“rest first”.This treatment principle will definitely play a positive role in improving clinical efficacy and provide ideas for clinical anti-tumor metastasis treatment using integrated traditional Chinese and Western medicine.
