基于数据挖掘仝小林应用穿山龙治疗甲状腺疾病的药物配伍规律及安全性分析

摘　　要：目的总结探析仝小林院士应用穿山龙治疗甲状腺疾病的药物配伍规律,为穿山龙治疗甲状腺疾病提供参考。方法收集2015年1月—2023年1月在仝小林院士门诊就诊的患有甲状腺疾病且处方内含有穿山龙患者的病历,采用SPSS Statistics 26软件进行频数统计,SPSS Modeler 18.0软件进行关联规则分析和SPSS Statistics26软件的系统聚类方法进行聚类分析及对患者应用穿山龙治疗前后的肝功能进行统计分析,分析仝小林院士应用穿山龙治疗甲状腺疾病的药物配伍规律及安全性分析。结果对仝小林院士使用穿山龙治疗甲状腺疾病常用的药物配伍经验进行分析总结,频数统计显示仝小林院士在治疗甲状腺疾病时用药以温、寒、平之性为主;药味以甘、苦、辛为主;用药归经以肝经为主,其次为脾、肺、肾、胃等,关联规则分析显示在治疗甲状腺疾病时最常用的靶方为穿山龙-三七-莪术-浙贝母;在不同原因所导致的甲状腺功能减退中用穿山龙-黄芪-当归-淫羊藿治疗气血亏虚、阳气不足之虚态,态靶同调;以穿山龙-夏枯草-牡蛎-五味子为治疗甲状腺结节之靶方,聚类分析得出仝小林院士常用来与穿山龙相配伍治疗甲状腺疾病的常用药物配伍。结论穿山龙药性平和,靶向明确,仝小林院士在治疗甲状腺疾病时擅长运用“态靶辨治”的思想,通过不同配伍将穿山龙用于甲状腺疾病的治疗,且对于肝功能没有不良影响,对临床应用穿山龙治疗甲状腺疾病具有一定的指导意义。Objective To summarize and analyze the drug compatibility rule of Professor TONG Xiaolin's application of Chuanshanlong(Dioscoreae Nipponicae Rhizoma)in the treatment of thyroid diseases,and to provide reference for Chuanshanlong(Dioscoreae Nipponicae Rhizoma)in the treatment of thyroid diseases.Methods From January 2015 to January 2023,medical records of patients with thyroid disease and prescription containing Chuanshanlong(Dioscoreae Nipponicae Rhizoma)in Professor TONG Xiaolin's outpatient department were collected.SPSS Statistics 26 software was used for frequency statistics.SPSS Modeler 18.0 software was used for association rule analysis and SPSS Statistics 26 software was used for systematic clustering analysis and statistical analysis of patients'liver function before and after Chuanshanlong(Dioscoreae Nipponicae Rhizoma)treatment,as well the drug compatibility rule and security of Academician TONG Xiaolin's application of Chuanshanlong(Dioscoreae Nipponicae Rhizoma)in the treatment of thyroid diseases was analyzed.Results Professor TONG Xiaolin's experience of using Chuanshanlong(Dioscoreae Nipponicae Rhizoma)in the treatment of thyroid diseases was analyzed and summarized.The frequency statistics showed that Professor TONG Xiaolin used warm,cold and calm drugs in the treatment of thyroid diseases.The taste of medicine is mainly sweet,bitter and xin.Liver channel was the main channel,followed by spleen,lung,kidney,stomach,etc.Association rule analysis showed that the most commonly used targets in the treatment of thyroid diseases were Chuanshanlong(Dioscoreae Nipponicae Rhizoma)-Sanqi(Notoginseng Radix Et Rhizoma)-Ezhu(Curcumae Rhizoma)-Zhebeimu(Fritillariae Thunbergii Bulbus)in hypothyroidism caused by different reasons,Chuanshanlong(Dioscoreae Nipponicae Rhizoma)-Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix)-Yinyanghuo(Epimedii Wushanensis Folium)was used to treat the deficiency of Qi and blood,and the deficiency of Yang-Qi.Taking Chuanshanlong(Dioscoreae Nipponicae Rhizoma)-Xiakucao(Prun
