摘 要:骨关节炎(osteoarthritis,OA)作为最常见的肌肉骨骼疾病之一,其较高的复发率及致残率给社会和家庭带来沉重负担,加之病程长及预后差的特点,仍是目前研究的热点问题,目前关于OA的发病机制仍不明确,多数学者认为骨关节炎与细胞凋亡及自噬有关,同时信号通路就是其最好的连接媒介。相关研究发现信号通路在疾病过程中的作用尤为突出,其中磷脂酰肌醇3-激酶/蛋白激酶B/哺乳动物雷帕霉素靶蛋白(phosphatidylinositol 3-kinase/proteinkinase B/mammalian target of rapamycin,PI3K/Akt/mTOR)信号通路尤为关键,该信号通路通过参与疾病发展过程中的细胞增殖、细胞凋亡及自噬等方面,影响着疾病的发生、发展,同时基于细胞自噬与凋亡层面,调控该信号通路能够对OA起到很好的治疗效果。既往研究表明,中药能够直接或间接通过调控PI3K/Akt/mTOR信号通路,抑制细胞自噬与凋亡的表达,以此来达到治疗OA的目的。因此,该文通过对近年来中药调控PI3K/Akt/mTOR信号通路表达,抑制细胞凋亡与自噬相关研究进展进行总结,旨在为应用中药防治因细胞凋亡与自噬引起的OA提供新的治疗思路。Osteoarthritis(OA)as one of the most common musculoskeletal diseases,its high recurrence rate and disability rate,bring serious burden to the society and family,combined with the long course and poor prognosis is still the research hotspot and key issues,the pathogenesis of OA is still not clear,most scholars believe that osteoarthritis is related to apoptosis and autophagy,at the same time the signal pathway is the best medium.Relevant studies have highlighted the role of signaling pathways in participating in disease processes,among them,phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin(PI3K/Akt/mTOR)signaling pathway is particularly critical,this signaling pathway through participating in cell proliferation,apoptosis and autophagy during the development of the disease,affaffects the development of the disease,based on the level of autophagy and apoptosis,regulation of this signaling pathway can achieve good results in treating OA.Previous studies showed that TCM can directly or indirectly inhibit the expression of cell autophagy and apoptosis by regulating the PI3K/Akt/mTOR signaling pathway,so as to achieve the purpose of treating OA.Therefore,this paper summarizes the progress of apoptosis regulation of PI3K/Akt/mTOR and inhibition of apoptosis and autophagy in recent years,aiming to provide a new therapeutic idea for traditional Chinese medicine to prevent OA caused by apoptosis and autophagy.
