基于异病同治探究葛根芩连汤历史沿革及现代临床应用

摘　　要：葛根芩连汤最早记载于东汉张仲景的《伤寒论》中,由葛根、黄芩、黄连、甘草4味中药组成,该方以清热止利为主,具有解表清里之功,为表里双解之剂,适用于太阳表邪内陷所致邪热下利证。随着历代医家的临床经验积累,葛根芩连汤的治疗范围不断扩大。“异病同治”是在中医辨证思想基础上的延伸法则,常见于治疗不同疾病相同或相似的病机,反映了中医辨证论治的特点。该文基于异病同治指导思想,对相关本草古籍中有关葛根芩连汤医籍记载情况、组成、煎服法及主治病证进行考证和统计,并梳理其历史沿革。同时对其现代临床应用研究进行筛选与归纳,发现葛根芩连汤治疗疾病范围远超古籍记载,其所治疾病的中医病机总属“湿热蕴结”,充分体现了异病同治的特点。综上,作者通过整理葛根芩连汤古今文献并加以分析,以期为该方的深入研究与临床应用提供思路与参考。Gegen Qinlian Decoction(葛根芩连汤)was first recorded in Treatise on Febrile Diseases,and consisted of four herbs,Gegen(Puerariae Lobatae Radix),Huangqin(Radix Scutellariae),Huanglian(Coptidis Rhizoma)and Gancao(Radix Glycyrrhizae).This formula is mainly used to clear heat and stop diarrhea,with the function of relieving the exterior and clearing the interior,and is a remedy for both exterior and interior.It is suitable for the syndrome of dysentery and diarrhea with heat caused by the inward sinking of Taiyang surface evil and the influence of evil heat.With the accumulation of clinical experience,the scope of treatment of Gegen Qinlian Decoction has been expanding.“Same treatment for different diseases”is an extended rule based on the dialectical thought of Chinese traditional medicine(TCM),commonly used in the treatment of different diseases with same or similar pathologies,reflecting the characteristics of syndrome differentiation and treatment of TCM.Base on the guiding principle of treating different diseases with the same treatment,the records,composition,decoction method and main symptoms of Gegen Qinlian Decoction in the ancient books of related materia medica were researched and statistically analyzed,and its historical evolution was sorted out.At the same time,the modern clinical application research was screened and summarized.It was found that the range of disease treated by Gegen Qinlian Decoction was far beyond that recorded in ancient books and the pathogenesis of TCM was“damp-heat accumulation”,which fully reflected the characteristic of the same treatment for different diseases.In summary,the ancient and modern literature of Gegen Qinlian Decoction was collected and analyzed in order to provide ideas and references for the in-depth research and clinical application of this prescription.
