基于“情志致病”理论探讨心理应激对于卵巢储备功能减退不孕影响

摘　　要：不孕症已成为世界性生殖健康问题,在其诊断及治疗的过程中常伴有焦虑、抑郁等不良情绪,而不良情绪作为一种心理应激亦加重不孕进程,卵巢储备功能减退(diminished ovarian reserve,DOR)是导致不孕的众多原因之一。研究显示,心理因素是促进卵巢储备功能减退的主要因素。“情志致病”理论在现代医学领域有较大的应用空间,其核心为肝失疏泄。结合现代医学的心理应激理论,该文分别从卵泡发育、排卵功能、子宫内膜容受性等方面阐述心理应激对不孕的影响,讨论心理干预在改善DOR不孕症患者妊娠结局中的作用,强调在个体化用药的同时关注患者的心理状况,必要时给予心理干预,减少患者的不良情绪,为DOR不孕症患者临床治疗提供新思路。Infertility has become a worldwide reproductive health problem.The progress of the diagnosis and treatment is often accompanied by anxiety,depression and other adverse emotions,and as a psychological stress,negative emotions also aggravate the infertility process.The diminished ovarian reserve(DOR)is one of the many causes of infertility.Through research,psychological factors are the main factors to promote the decline of ovarian reserve function.The theory of“emotion causing disease”has great application space in the field of modern medicine,and its core is that the liver loses its function of dispersing Qi.Combined with the psychological stress theory of modern medicine,this paper expounds the influence of psychological stress on infertility from the aspects of follicular development,ovulation function,endometrial receptivity,and other aspects,discusses the role of psychological intervention in improving the pregnancy outcome of DOR infertility patients,and emphasizes paying attention to patients'psychological status while individualized medication,and giving psychological intervention when necessary to reduce patients'bad emotions,which provides new ideas for clinical treatment of DOR infertility patients.
