摘　　要：慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)已成为世界第三大致死性疾病。全球COPD病死率持续升高,严重威胁人们的生活质量和健康状况。其中,中国患者病死率及疾病负担高于世界水平。而COPD合并症是导致疾病急性加重、再住院和死亡的主要原因,尤其是合并心血管疾病(cardiovascular disease,CVD)是伤残率及急性加重死亡的重要原因。目前,临床上对于COPD合并心血管疾病的病因病理机制研究、相关治疗药物是否会引起疾病的发生、是否会导致患者死亡风险增加的研究甚少。因此,该文从吸烟、氧化应激、缺氧、血管功能障碍、血小板过度活化、蛋白酶-抗蛋白酶及交感神经的角度探索COPD合并心血管疾病的病理机制。同时,针对COPD及心血管疾病常用治疗药物,如他汀类、β受体阻断剂、胆碱能拮抗剂及茶碱类等药物上探索双方的关系,减少COPD合并心血管疾病的发生、发展,降低患者经济负担,降低病死率,提高患者生活质量及生存率。Chronic obstructive pulmonary disease(COPD)has become the third leading cause of death worldwide,with an increase in the global mortality rate of COPD,posing a serious threat to people's quality of life and health.Among them,the mortality rate and disease burden of Chinese patients are higher than the global level.Moreover,comorbidities of COPD are the main causes of acute exacerbations,rehospitalization,and death in COPD,especially COPD complicated with cardiovascular disease(CVD),which is a significant contributor to the disability rate and acute exacerbation-related mortality of COPD.Currently,there is a paucity of research on the etiopathogenesis of COPD complicated with CVD and whether COPD treatment drugs and cardiovascular drugs can induce the occurrence of COPD complicated with CVD or increase the risk of death.Therefore,this article explores the pathological mechanisms of COPD complicated with CVD from the perspectives of smoking,oxidative stress,hypoxia,vascular dysfunction,platelets,protease-antiprotease,and sympathetic nerves.Additionally,this article delves into the relationship between common therapeutic drugs for COPD and CVD,such as statins,beta-blockers,cholinergic antagonists,and xanthines,aiming to reduce the occurrence and progression of COPD complicated with CVD,alleviate the economic burden on patients,lower mortality rates,and enhance patients'quality of life and survival rates.
