摘 要:目的 探讨神经元特异性烯醇化酶(NSE)与中性粒细胞百分率和白蛋白比值(NPAR)在脓毒症相关性谵妄(SAD)中的变化及意义。方法 收集2022年9月至2023年9月在孝感市中心医院ICU住院的脓毒症患者资料,根据ICU意识模糊评估法(CAM-ICU)将患者分为SAD组和N-SAD组,收集患者一般临床资料、实验室指标结果,应用Logistic回归模型分析SAD发生的独立危险因素。结果 纳入的90例脓毒症患者中,其中SAD患者39例,SAD发生率43.33%。两组急性生理与慢性健康状况评分系统(APACHEⅡ)评分、血红蛋白(Hb)、白细胞(WBC)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、血小板压积(PCT)、肌钙蛋白Ⅰ(cTnⅠ)、D-二聚体(D-D)、神经元特异性烯醇化酶(NSE)、中性粒细胞百分率/白蛋白比值(NPAR)比较,差异有统计学意义(P<0.05)。将单因素分析有统计学差异的指标进行多因素Logistic回归分析,结果表明APACHEⅡ评分、NSE、NPAR是SAD的独立危险因素(P<0.05)。结论 NPAR、NSE是SAD的独立危险因素,可作为SAD的早期预测指标。【Objective】 To investigate the changes of neuron-specific enolase(NSE) and neutrophil percentage-to-albumin ratio(NPAR) in sepsis-associated delirium(SAD) and their significance.【Methods】 The data of sepsis patients hospitalised in the ICU of Xiaogan Central Hospital from September 2022 to September 2023 were collected,and the patients were divided into SAD group and N-SAD group according to the ICU Confusion Assessment Method(CAM-ICU),the general clinical data and laboratory index results of the patients were collected,and the independent risk factors for SAD were analyzed by logistic regression model.【Results】 Among the 90 patients with sepsis,39 were SAD,and the incidence of SAD was 43.33%.There were statistically significant differences in APACHE II score,Hb,WBC,ALT,AST,PCT,cTnI,DD,NSE,NPAR between the two groups(P<0.05).The results of multivariate logistic regression analysis showed that APACHE II,NSE,NPAR were independent risk factors for SAD(P<0.05).【Conclusion】 The values of NPAR and NSE in the SAD group were significantly higher than those in the N-SAD group,and NPAR and NSE were independent risk factors for SAD and could be used as early predictors of SAD.
