针灸八髎穴治疗女性盆底功能障碍性疾病的系统评价

摘　　要：目的探讨针灸八髎穴治疗女性盆底功能障碍性疾病的临床疗效。方法通过检索2010年1月至2023年10月国内外权威医学杂志的全文数据库,筛选出针刺八髎穴联合疗法治疗女性盆底功能障碍性疾病的随机对照试验(RCT)。采用Cochrane方法对入选文献进行偏倚危险度评估,并采用RevMan5.3软件进行统计分析。结果共纳入13个RCT,涉及938例患者。Meta分析结果显示,针灸八髎穴在生活质量量表评分[MD=5.74,95%CI(-4.86~16.34)]、性生活质量评分[MD=7.19,95%CI(-6.36~20.75)]、膀胱残余尿量[MD=-0.18,95%CI(-11.5~11.13)]、快肌阶段盆底肌表面肌电位[SMD=0.41,95%CI(-0.33~1.15)]、慢肌阶段盆底肌表面肌电位[SMD=3.31,95%CI(-2.36~8.97)]方面与其他盆底康复疗法相比差异无统计学意义(P>0.05)。然而,在肛管直肠压力静息压[MD=-2.67,95%CI(-4.24~-1.10)]、尿失禁量[MD=-2.99,95%CI(-3.19~-2.79)]、尿失禁问卷简表[MD=-2.61,95%CI(-3.15~-2.07)]及漏尿量[SMD=-1.61,95%CI(-2.12~-1.11)]方面,针灸八髎穴联合疗法优于其他盆底康复疗法,差异有统计学意义(P<0.05)。结论针灸八髎穴是治疗女性盆底功能障碍的有效方法,具有双向调节作用。Objective To explore the clinical efficacy of acupuncture at the Baliao acupoints in treating female pelvic floor dysfunction diseases.Methods By searching the full-text databases of domestic and international authoritative medical journals from January 2010 to October 2023,we selected randomized controlled trials(RCTs)of combined therapy with acupuncture at the Baliao acupoints for female pelvic floor dysfunction diseases.The Cochrane method was used to assess the risk of bias in the included literature,and RevMan 5.3 software was used for statistical analysis.Results A total of 13 RCTs involving 938 patients were included.Meta-analysis results showed that acupuncture at the Baliao acupoints did not significantly differ from other pelvic floor rehabilitation therapies in terms of quality of life score[MD=5.74,95%CI(-4.86~16.34)],sexual quality of life score[MD=7.19,95%CI(-6.36~20.75)],residual urine volume in the bladder[MD=-0.18,95%CI(-11.5~11.13)],fast-twitch stage pelvic floor muscle surface electromyogram[SMD=0.41,95%CI(-0.33~1.15)],and slow-twitch stage pelvic floor muscle surface electromyogram[SMD=3.31,95%CI(-2.36~8.97)](P>0.05).However,acupuncture combined therapy at the Baliao acupoint was superior to other pelvic floor rehabilitation therapies in terms of anal rectal pressure at rest[MD=-2.67,95%CI(-4.24~-1.10)],urinary incontinence volume[MD=-2.99,95%CI(-3.19~-2.79)],urinary incontinence questionnaire short form[MD=-2.61,95%CI(-3.15~-2.07)],and urine leakage volume[SMD=-1.61,95%CI(-2.12~-1.11)],with statistically significant differences(P<0.05).Conclusion Acupuncture at the Baliao acupoints is an effective method for treating female pelvic floor dysfunction,with a dual regulatory effect.
