医院儿科病房耐甲氧西林金黄色葡萄球菌分子特征与耐药性分析

摘  要:目的调查儿童患者中耐甲氧西林金黄色葡萄球菌(MRSA)在呼吸道样本中的流行情况,评估其抗药性,并分析感染风险因素。通过分子流行病学分析,为疾病的源头追踪提供科学依据。方法收集2020年1月至2021年6月本院儿童患者呼吸道标本中分离出的金黄色葡萄球菌277株。采用χ2检验和非条件Logistic回归分析MRSA感染的危险因素,并检测耐药基因mecA、mecC以及主要毒力基因杀白细胞毒素(PVL)。同时,进行分子生物学分型。结果在277株金黄色葡萄球菌株中,检出MRSA共78株,占比28.16%,所有MRSA菌株均携带mecA基因。MRSA菌株中,最常见的分子分型为ST59-SCCmec IV-t437(39.74%),其次为ST398-SCCmecⅤ-t034(11.54%),其他克隆型占比均小于10%。结论MRSA表现出多药耐药的特征,医院需要加强MRSA分子特征监测,防止MRSA的克隆传播。Objective To investigate the prevalence of methicillin-resistant Staphylococcus aureus(MRSA)in respiratory samples from pediatric patients,assess its antibiotic resistance,and analyze risk factors for infection.Molecular epidemiological analysis was conducted to provide scientific support for tracing the source of the disease.Methods A total of 277 Staphylococcus aureus strains isolated from respiratory specimens of pediatric patients in our hospital from January 2020 to June 2021 were collected.Chi-square(χ2)tests and unconditional logistic regression were used to analyze the risk factors for MRSA infection,and resistance genes mecA and mecC,as well as the main virulence gene Panton-Valentine leukocidin(PVL),were detected.Molecular biological typing was also performed.Results Among the 277 Staphylococcus aureus strains,78 were identified as MRSA,accounting for 28.16%,and all MRSA strains carried the mecA gene.The most common molecular type among MRSA strains was ST59-SCCmec IV-t437(39.74%),followed by ST398-SCCmec V-t034(11.54%),with other clone types accounting for less than 10%.Conclusion MRSA exhibits characteristics of multidrug resistance,and hospitals should strengthen the monitoring of MRSA molecular characteristics to prevent the clonal spread of MRSA.
