摘 要:目的探讨罗沙司他对重组人促红细胞生成素(rhEPO)低反应性肾性贫血患者的疗效,并分析其对患者铁代谢和炎症指标的影响。方法选取2022年1月至2024年3月本院接受治疗的283例肾性贫血患者作为研究对象,分为rhEPO组(98例)、罗沙司他组(98例)和rhEPO+罗沙司他组(87例),所有患者均接受规律血液透析治疗。在药物治疗前、中和后监测红细胞(RBC)、血红蛋白(Hb)、血细胞比容(Hct)、血清铁蛋白(SF)、转铁蛋白饱和度(TSAT)、转铁蛋白(TRF)、C反应蛋白(CRP)、白介素-6(IL-6)等指标。结果rhEPO+罗沙司他联合治疗组的总有效率优于rhEPO组和罗沙司他组,CRP及IL-6低于rhEPO组和罗沙司他组(P<0.05)。三组患者治疗前的基线水平均衡(P>0.05);治疗后,三组患者的RBC、Hb、Hct、SF、TSAT、TRF均较治疗前升高(P<0.05)。结论罗沙司他联合rhEPO治疗效果优于单药治疗,患者的贫血情况、铁代谢指标和炎症指标均得到显著改善。Objective To investigate the efficacy of Roxadustat in patients with renal anemia who have a low response to recombinant human erythropoietin(rhEPO),and to analyze its impact on iron metabolism and inflammatory markers.Methods A total of 283 patients with renal anemia treated at our hospital from January 2022 to March 2024 were selected as the study subjects,divided into three groups:the rhEPO group(98 cases),the Roxadustat group(98 cases),and the rhEPO combined with Roxadustat group(87 cases).All patients received regular hemodialysis treatment.Hemoglobin(Hb),red blood cell count(RBC),hematocrit(Hct),serum ferritin(SF),transferrin saturation(TSAT),transferrin(TRF),C-reactive protein(CRP),and interleukin-6(IL-6)were monitored before,during,and after drug treatment.Results The overall response rate of the rhEPO with Roxadustat combination therapy group was higher than that of the rhEPO group and the Roxadustat group,with CRP and IL-6 levels lower than those in the rhEPO group and the Roxadustat group(P<0.05).The baseline levels of the three groups were balanced before treatment(P>0.05).After treatment,RBC,Hb,Hct,SF,TSAT,and TRF in all three groups increased compared to before treatment(P<0.05).Conclusion The combination therapy of Roxadustat with rhEPO shows superior efficacy compared to monotherapy,with significant improvements in anemia,iron metabolism indicators,and inflammatory markers in patients.
