固崩止漏汤联合地屈孕酮治疗对围绝经期异常子宫出血患者子宫内膜厚度及血清性激素水平的影响

摘　　要：目的探讨固崩止漏汤联合地屈孕酮治疗对围绝经期异常子宫出血(AUB)患者的影响。方法选取2022年3月—2024年3月济宁市兖州区人民医院收治的90例围绝经期AUB患者为研究对象,按随机数字表法将其分为对照组和观察组,各45例。对照组采用地屈孕酮治疗,观察组在对照组基础上加用固崩止漏汤治疗。比较两组的临床疗效、血红蛋白水平、子宫内膜厚度、血清性激素水平及不良反应发生情况。结果观察组治疗总有效率为93.33%,高于对照组的77.78%,差异有统计学意义(P<0.05);治疗后,观察组血红蛋白水平高于对照组,子宫内膜厚度薄于对照组,雌二醇、孕酮、黄体生成素、卵泡生成激素水平均低于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论固崩止漏汤联合地屈孕酮治疗围绝经期AUB患者的临床效果显著,可改善子宫内膜厚度,降低血清性激素水平,且安全性高。Objective To investigate the effect of Gubeng Zhilou decoction combined with dydrogesterone on abnormal uterine bleeding(AUB)during perimenopausal period.Methods A total of 90 patients with perimenopausal AUB admitted to Yanzhou District People's Hospital of Jining from March 2022 to March 2024 were selected as the research objects,according to the random number table method,they were divided into a control group and an observation group,with 45 cases in each group.The control group was treated with dydrogesterone,and the observation group was treated with Gubeng Zhilou decoction on the basis of the control group.The clinical efficacy,hemoglobin level,endometrial thickness,serum sex hormone level and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.33%,which was higher than 77.78%of the control group,the difference was statistically significant(P<0.05).After treatment,the hemoglobin level of the observation group was higher than that of the control group,the endometrial thickness was thinner than that of the control group,and the levels of estradiol,progesterone,luteinizing hormone and follicle-generating hormone were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Gubeng Zhilou decoction combined with dydrogesterone in the treatment of perimenopausal AUB has significant clinical effect,which can improve endometrial thickness,reduce serum sex hormone levels,and has high safety.
