摘 要:目的讨论乳腺影像报告和数据系统(BI-RADS)分类标准联合剪切波弹性成像(SWE)对高原地区乳腺良恶性病变的诊断价值。方法收集来自高原地区的共196个乳腺病变行常规超声检查,运用BI-RADS分类进行良恶性判断,并运用剪切波弹性成像检查获得95个病变的弹性定量参数,以病理结果为金标准,构建ROC曲线,比较两种方法单独运用以及二者联合运用的诊断价值。结果BI-RADS分类、SWE以及二者联合诊断高原地区乳腺良恶性病变的曲线下面积分别为0.888、0.904、0.938,二者联合运用的曲线下面积大于BI-RADS分类或SWE单独运用的曲线下面积。二者联合运用后,减少了BI-RADS分类的误判,提高了诊断的准确度,减少了不必要的有创操作。结论BI-RADS分类联合SWE诊断高原地区乳腺良恶性病变的价值高于两种方法单独使用,值得在高原地区推广。Objective To explore the diagnostic value of the breast imaging reporting and data system(BI-RADS)combined with shear wave elastography(SWE)in benign and malignant breast lesions in the plateau area.Methods A total of 196 breast lesions in the patients from plateau areas were collected for routine ultrasound examination,and BI-RADS category was used to judge benign and malignant;shear wave elastography was used to obtain elastic quantitative parameters of 95 lesions,and the pathological results were used as the gold standard.The ROC curve was constructed to comapre the diagnostic value of the two methods alone and their combination.Results The areas under the curve for diagnosis of benign and malignant breast lesions in plateau areas by BI-RADS category,SWE and their combination were 0.888,0.904 and 0.938,respectively.The areas under the curve for the combination of the two were larger than those for the two alone.The combination of the two reduces misjudgments of BI-RADS category,improves the accuracy of diagnosis,and reduces unnecessary invasive operations.Conclusions The value of BI-RADS category combined with SWE in diagnosing benign and malignant breast lesions in plateau areas is higher than that of the two methods alone,and is worth promoting in plateau areas.
