摘 要:目的探讨实时剪切波弹性成像(SWE)评估冻胚移植患者(FET)子宫内膜容受性(ER)的价值。方法回顾性选取FET患者共72例,根据临床妊娠结局将研究人群分为妊娠组和非妊娠组,收集并比较两组的一般临床资料、移植前一日常规超声参数及内膜平均弹性模量值。结果两组间一般临床资料、内膜厚度、容积、子宫动脉搏动指数(PI)差异均无统计学意义(P>0.05)。两组间子宫螺旋动脉搏动指数(PI)、内膜及内膜下血流分支及内膜平均弹性模量值差异有统计学意义(P<0.05)。子宫螺旋动脉PI值、内膜平均弹性模量值是妊娠结局的独立影响因素(P<0.05)。子宫螺旋动脉PI值、内膜平均弹性模量值预测妊娠结局的曲线下面积分别为0.720、0.787。二者联合预测妊娠结局的曲线下面积为0.836,比单一指标预测价值更高。结论SWE能有效评价FET患者ER,SWE测得的内膜平均弹性模量值对FET患者妊娠结局有一定的预测价值,与子宫螺旋动脉PI值联合时可进一步提高预测效能。Objective To explore the value of real-time shear wave elastography(SWE)in evaluating endometrial receptivity(ER)in frozen embryo transfer patients(FET).Methods In this retrospective study,72 patients who had received FET were selected and were divided into a pregnancy group and a non-pregnancy group based on their clinical pregnancy outcomes.General clinical data,routine ultrasound parameters one day before the transfer,and the mean endometrial elasticity modulus values were collected and compared between the two groups.Results The results showed that there was no statistically significant difference in the general clinical data,endometrial thickness,endometrial volume,and uterine artery pulsatility index(PI)between the two groups(P>0.05).There was a statistically significant difference in the uterine spiral artery PI,endometrial and subendometrial blood flow branches,and the average elastic modulus of the endometrium between the two groups(P<0.05).PI value of spiral artery and mean elastic modulus of endometrium were independent factors of pregnancy outcome(P<0.05).The area under the curve(AUC)for predicting pregnancy outcomes using uterine spiral artery PI and mean endometrial elasticity modulus were 0.720 and 0.787,respectively.The prediction model with both parameters had an AUC of 0.836,higher than that of either single indicator.Conclusions SWE can effectively evaluate ER in patients with FET,and the average elastic modulus of endometrium measured by SWE can predict the pregnancy outcome of patients with FET to a certain extent,which can be further improved when combined with PI value of uterine spiral artery.
