摘　　要：目的回顾性分析盆腔静脉内平滑肌瘤病(IVL)的临床资料和超声误诊原因,以提高其术前诊断准确率和手术成功率。方法收集术后组织病理学诊断为静脉内平滑肌瘤病的32例患者的临床资料,总结超声特征。结果患者中位年龄41.5岁,术前确诊11例(34.4%),误诊21例(65.6%)。主要误诊原因:对静脉内平滑肌瘤病超声表现细节认识不足,忽视合并的子宫旁血管内病灶。结论静脉内平滑肌瘤病临床表现缺乏特异度易误诊和漏诊。超声表现具有一定特征性,可作为术前诊断和术后长期随访的首选检查,临床医师和超声医师通过加强对该罕见病的认识,有利于获得良好的治疗结局。Objective Retrospective analysis of the clinical data and causes of misdiagnosis by ultrasound of pelvic intravenous leiomyomatosis(IVL)is aimed at improving the preoperative diagnostic accuracy and surgical success rate.Methods The study collected the clinical data of 32 patients with IVL who underwent postoperative histopathological diagnosis,as well as the ultrasonic characteristics.Results The median age of patients was 41.5 years,11 cases(34.4%)were diagnosed and 21 cases(65.6%)were misdiagnosed before operation.The main reasons for misdiagnosis:The details of IVL ultrasound manifestations were not well understood,and the intravascular lesions beside the uterus was ignored.Conclusions The IVL lacks of specificity in clinical manifestations,and are prone to misdiagnosis and missed diagnosis.The ultrasonic manifestations have certain characteristics and can be used as the first choice for preoperative diagnosis and long-term postoperative follow-up.By strengthening the understanding of this rare disease,clinicians and sonographers are conducive to a good treatment outcome.
