摘 要:目的探讨超声定量参数在诊断肩袖撕裂风险中的价值。方法将110例肩关节疼痛患者分为肩袖撕裂组和非撕裂组,比较两组患者的超声定量参数,包括肩峰-三角肌下滑囊厚度(SASDBT)、冈上肌腱厚度(SSTT)、中立位及外展60°肩肱距离(AHD)、中立位及外展60°肩峰肱骨大结节距离(AGTD)、中立位喙肱距离(CHD)及SSTT/AHD。结果与非撕裂组相比,撕裂组患者的SSTT/AHD较大,外展60°AHD较小,差异有统计学意义(P<0.05)。SSTT/AHD以51.97%为截断值预测冈上肌腱撕裂风险的灵敏度为80.0%,特异度为68.7%,曲线下面积为0.785。外展60°AHD以7.45 mm为截断值预测冈上肌腱撕裂风险的灵敏度为86.7%,特异度为30.0%,曲线下面积为0.554。结论SSTT/AHD增大、外展60°AHD缩小是肩袖撕裂的危险因素,超声可作为评估肩袖撕裂风险的新方法。Objective To explore the value of ultrasonic quantitative parameters in diagnosing the risk of rotator cuff tear.Methods A total of 110 patients with shoulder pain were enrolled and divided into the rotator cuff tear group and the non-tear group.The ultrasound quantitative parameters,including subacromial-subdeltoid bursa thickness(SASDBT),supraspinatus tendon thickness(SSTT),acromiohumeral distance(AHD)at neutral position and 60°abduction,acromiogreater tuberosity distance(AGTD)at neutral position and 60°abduction,coracohumeral distance(CHD)at neutral position,and the ratio of SSTT to AHD(SSTT/AHD),were analyzed between the two gro ups.Results Compared with the non-tear group,the SSTT/AHD ratio was significantly higher,and the AHD at 60°abduction was significantly lower in the tear group(P<0.05).Using a cut-off value of 51.97% for SSTT/AHD,the sensitivity for predicting the risk of supraspinatus tendon tears was 80.0%,the specificity was 68.7%,and the area under the receiver operating characteristic curve(AUC)was 0.785.With a cut-off value of 7.45 mm for AHD at 60°abduction,the sensitivity for predicting the risk of supraspinatus tendon tears was 86.7%,the specificity was 30.0%,and the AUC was 0.554.Conclusions The increase of SSTT/AHD ratio and the decrease of AHD in abduction 60°are risk factors for rotator cuff tear,and ultrasound can be used as a novel approach for assessing the risk of rotator cuff tear.
