摘 要:目的探讨正常成人下桡尺关节(DRUJ)超声扫查方法及声像图特点。方法以MRI图像为参考,对60例正常成人DRUJ行超声检查,采用13个标准切面,记录并描述其内部结构的声像图特征。结果(1)DRUJ间隙:横切面呈“缝隙状”、纵切面呈“片状”无回声,尺骨头呈“圆弧状”,桡骨背面呈“平面状”或“浅凹状”。(2)三角纤维软骨(TFC):冠状面呈“束腰状”,横切面呈“半圆形”。(3)半月板同系物:冠状面呈“倒三角形”或“鱼尾形”。(4)掌、背侧桡尺韧带:桡侧段呈“弧形”包绕TFC,尺侧段呈“Y形”分为深、浅两支。(5)尺三角、尺月、尺侧副韧带:呈粗细均匀“条带状”。(6)尺侧腕伸肌腱腱鞘:呈“环状”包绕尺侧腕伸肌腱。结论超声能清晰辨识DRUJ内部结构,为临床诊断提供新参考。Objective To explore the ultrasound scanning methods and sonographic characteristics of the distal radioulnar joint(DRUJ)in normal adults.Methods Using MRI images as a reference,ultrasonography was performed on 60 normal adult DRUJs.Using MRI images as a reference,13 standard sections were selected to record and describe the sonographic characteristics of their internal structures.Results(1)DRUJ space:it apperared as a“slit-like”anechoic area in the transverse section,and a“sheet-like”anechoic area in the longitudinal section.The head of the ulna was“arc-shaped”,while the dorsal of the radius was“flat”or“shallowly concave”.(2)Triangular fibrocartilage(TFC):it presented a“waisted”shape in the coronal section and a“semicircular”shape in the transverse section.(3)Ulnomeniscal homologue:in the coronal section,it appeared as an“inverted triangle”or“fish tail”shape.(4)Radioulnar ligaments on palmar and dorsal sides:the radial segments curved around TFC in an“arc”shape,while the ulnar segments were divided into deep and superficial branches in a“Y”shape.(5)Ulnotriquetral ligament,ulnolunate ligament,and ulnar collateral ligament all appeared as uniform“banded”structures in thickness.(6)Sheath of extensor carpi ulnars tendon:it encircled the extensor carpi ulnars tendon in a“ring-like”manner.Conclusions Ultrasound can clearly identify the internal structures of DRUJ,providing a new reference for clinical diagnosis.
