自体表皮移植联合白芍总苷胶囊及富血小板血浆治疗稳定期白癜风的效果研究

摘　　要：目的观察自体表皮移植联合白芍总苷胶囊及富血小板血浆(PRP)皮内注射治疗稳定期白癜风的临床效果。方法选取2020年6月至2023年3月收治的82例稳定期白癜风患者,采用随机数字表法分为2组。观察组42例采用自体表皮移植联合白芍总苷胶囊术前1个月、术后3个月口服及PRP皮内注射治疗,治疗周期内共完成8次PRP皮内注射;对照组40例采用自体表皮移植联合白芍总苷胶囊术前1个月及术后3个月口服治疗。2组均在治疗12周后随访6个月以评估治疗效果,并比较治疗前后皮肤病生活质量指数(DLQI)和不良反应发生率。结果观察组总有效率为92.86%(39/42)高于对照组的75.00%(30/40),差异有统计学意义(P<0.05)。治疗12周后及随访6个月后,观察组DLQI评分明显低于对照组(P<0.01)。2组总不良反应发生率比较差异无统计学意义(P>0.05)。结论自体表皮移植结合白芍总苷胶囊、PRP皮内注射治疗稳定期白癜风效果显著,不仅能有效促进白斑消退,还能改善患者生活质量,且安全性高。Objective To observe the clinical effect of autologous epidermal transplantation combined with total glucocides of paeony capsules and intradermal injection of platelet-rich plasma(PRP)in the treatment of stable vitiligo.Methods A total of 82 patients with stable vitiligo from June 2020 to March 2023 were selected and divided into two groups by random number table method.The 42 patients in the observation group were treated with autologous epidermal transplantation combined with oral administration of total glucosides of paeony capsules at 1 month before surgery and at 3 months after surgery and intradermal injection of PRP.During the treatment period,a total of 8 PRP intradermal injections were completed.Another 40 patients in the control group were treated with autologous epidermal transplantation combined with oral administration of total glycosides of paeony capsules at 1 month before surgery and at 3 months after surgery.Both groups were followed up for 6 months after 12 weeks of treatment to evaluate the therapeutic effect,and to compare the dermatological life quality index(DLQI)and the incidence of adverse reactions before and after treatment.Results The total effective rate was 92.86%(39/42)in the observation group,which was higher than that[75.00%(30/40)]in the control group,showing significant difference(P<0.05).At 12 weeks after treatment and 6 months after follow-up,DLQI score of observation group was significantly lower than that of control group(P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Autologous epidermal transplantation combined with total glycosides of paeony capsules and intradermal injection of PRP can effectively promote the regression of white spots and improve the quality of life of patients with stable vitiligo,showing high safety.
