摘  要:目的探究体质量指数(BMI)和血清特异性免疫球蛋白E(sIgE)水平与过敏性哮喘急性发作期患儿肺功能及疗效的关系。方法选取2022年1月至2024年1月儿科门诊就诊的过敏性哮喘急性发作期患儿120例,按照中国0~18岁儿童、青少年BMI生长曲线分为肥胖组30例、超重组30例和非肥胖组60例,根据血清sIgE水平分为强阳性组35例、阳性组57例和阴性组28例。所有患儿给予哮喘规范化治疗4个月。治疗后比较各组患儿治疗效果和治疗前后第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、呼气峰值流速(PEF)水平,并采用Pearson相关性分析法分析BMI、血清sIgE水平与肺功能的相关性。结果治疗后,肥胖组及超重组治疗总有效率低于非肥胖组(P<0.05),强阳性组及阳性组治疗总有效率低于阴性组(P<0.05)。治疗后肥胖组、超重组、非肥胖组及强阳性组、阳性组、阴性组FEV1、FVC、PEF均高于治疗前(P<0.05);超重组及肥胖组治疗后FEV1、FVC、PEF均低于非肥胖组,肥胖组治疗后FEV1、FVC、PEF均低于超重组(P<0.01)。强阳性组及阳性组治疗后FEV1、FVC、PEF均低于阴性组,强阳性组治疗后FEV1、FVC、PEF均低于阳性组(P<0.05)。Pearson相关性分析显示,过敏性哮喘急性发作期患儿BMI和血清sIgE水平与肺功能指标FEV1、FVC、PEF均呈负相关(P<0.01)。结论BMI和血清sIgE水平升高与过敏性哮喘急性发作期患儿肺功能受损有关,并影响其规范化治疗效果。Objective To explore the relationship of body mass index(BMI)and serum specific immunoglobulin E(sIgE)levels with pulmonary function and therapeutic effect in children with acute exacerbation of allergic asthma.Methods A total of 120 children with acute exacerbation of allergic asthma were selected from the pediatric outpatient department from January 2022 to January 2024.According to the growth curve of BMI of Chinese children and adolescents aged 0 to 18 years,they were divided into obesity group(n=30),overweight group(n=30)and non-obesity group(n=60).According to the total serum sIgE level,they were dibided into strongly positive group(n=35),positive group(n=57)and negative group(n=28).All children were given standardized treatment for asthma for 4 months.After treatment,the therapeutic effect and the levels of pulmonary function[the forced expiratory volume in one second(FEV1),forced vital capacity(FVC)and peak expiratory flow rate(PEF)]before and after treatment were compared in all groups.Pearson correlation analysis was used to analyze the correlation between BMI,serum sIgE and pulmonary function.Results After treatment,the total effective rate of obesity group and overweight group was lower than that of non-obesity group(P<0.05),and the total effective rate of strongly positive group and positive group was lower than that of negative group(P<0.05).After treatment,FEV1,FVC and PEF were higher than those before treatment(P<0.05).After treatment,FEV1,FVC and PEF in overweight and obesity groups were lower than those in non-obesity group,while FEV1,FVC and PEF in obesity group were lower than those in overweight group(P<0.01).After treatment,FEV1,FVC and PEF in strongly positive group and positive group were lower than those in negative group,while FEV1,FVC and PEF in strongly positive group were lower than those in positive group(P<0.05).Pearson correlation analysis showed that BMI and serum sIgE levels were negatively correlated with pulmonary function indexes FEV1,FVC and PEF in children with acute exacerb
