基于菌群-肠-肾轴探讨“治未病”思想防治糖尿病肾病

摘　　要：糖尿病肾病(diabetic kidney disease,DKD)患病率近年来呈不断上升趋势,威胁着人类健康。随着国内外对肠道微生态深入研究,“肠-肾轴”逐渐成为研究热点,为临床治疗各种肾脏病提供理论依据。“治未病”理论源于《黄帝内经》,包括未病先防、既病防变。文章将从菌群-肠-肾轴的角度探讨如何通过调节饮食、情志、运动及早期检测肠道菌群变化实现“未病先防”。结合“脾虚为病机之本”通过运脾以改善肠道微生态,实现“既病防变”,为临床防治DKD提供新的思路及治法。The prevalence of diabetic kidney disease(DKD)has been increasing in recent years,threatening human health.With the in-depth study of intestinal micro-ecology at home and abroad,the“gut-kidney axis”has gradually become a research hot-spot,providing a theoretical basis for the clinical treatment of various kidney diseases.The theory of“preventing disease before it occurs”originated from the Yellow Emperor’s Classic of Internal Medicine,including prevention before disease occurs and prevention of changes after disease occurs.From the perspective of the microbiota-gut-kidney axis,this paper discusses how to achieve“prevention before disease occurs”by adjusting diet,emotions,exercise and early detection of changes in intestinal flora.Combined with“spleen deficiency as the root of pathogenesis”,the spleen is used to improve intestinal micro-ecology to achieve“prevention of changes after disease occurs”,providing new ideas and treatment methods for the clinical prevention and treatment of DKD.
